About the role

:

In Swansea, we aim to provide adults and their carers with the right support, at the right time, by the right person. Our approach follows the Principles of the Social Services and Well Being Act (Wales) , which focuses on working with people, in partnership, to meet the clients needs and stop any issues from escalating. 

We are looking for the right candidate who is dynamic, has great organisational skills, is motivated, passionate and enthusiastic about the role, and will 'work alongside Swansea residents to live well and safely in our community'. 

This role is fast paced, and you will play an important role in assessing, planning, implementing, and evaluating care plans. Your passion for social work will enable effective integrated working, with vulnerable adults in Swansea, their families, groups and communities to enable optimum outcomes to promote independence and social inclusion.
