We are currently looking for an enthusiastic, highly motivated Specialist Physiotherapist in the newly developing Integrated Community Stroke Service within Sherwood Forest Hospitals NHS Foundation Trust.

The current Stroke Early Supported Discharge team was established in 2009 and since April 2023 has been able to expand and develop the service to be able to provide more access to specialist stroke community-based rehabilitation within the localities it covers. This high-profile community-based service covers Mansfield, Ashfield, Newark and Sherwood and receives referrals for patients following an acute stroke.

The service aims to facilitate a smooth and timely transition from hospital to home optimising stroke recovery, function and independence for the initial 6 months post discharge home from hospital.

The team has a strong ethos of patient centred care and high clinical standards. We are looking for a specialist Physiotherapist that has a keen interest in stroke care and rehabilitation and that is keen for a new challenge and would like opportunities for self and service development.

Full time and part time positions are available and can be discussed upon interview.

Main duties of the job

To provide a timely patient-centred, specialist stroke rehabilitation service responsive to individual need based in the community. To provide high quality service as part of a dedicated specialist stroke rehabilitation service with the local communities of Mansfield and Ashfield and Newark and Sherwood localities. Maximise independence and active lifestyle and minimise disability. Being proactive in facilitating independent living in the community wherever possible, by working with patients and their carers and other members of the team by setting realistic goals and evaluating their outcomes. To liaise with other health and social care services facilitating the provision of a seamless service for the patient. Training and education of physiotherapy, occupational therapy, medical, nursing and other professional staff/students as required.

Job responsibilities

Works as a lead therapist within the community rehabilitation team Undertakes individual patient care planning (including triage calls) and implements effective physiotherapy treatment, which is responsive to patients needs and at the same time supporting their equality, diversity and rights. Develops and maintain close, purposeful communication and liaison with wider rehabilitation teams to enhance the quality of patient care, to promote the effective use of resources and to ensure a collaborative and interdisciplinary approach to service provision. Take a lead in the audit of treatment protocols to evaluate the quality, effectiveness and efficiency of the service provision. Works within and being aware of all Health and Safety guidelines and policies related to manual handling, infection control, personal safety and risk management and shows due regard for others in the working environment. Ensures that problem orientated treatment records are maintained and updated to accurately reflect physiotherapy intervention, in line with trust policy and professional standards. To demonstrate highly developed dexterity, co-ordination and palpatory senses for assessment and manual treatment of patients. Communicate complex information to patients, carers and colleagues appropriately, sensitively and effectively taking into account the particular needs of the situation and the individuals. Evaluate own practice and identify own training and development needs in order to ensure continuing personal and professional development and competence to practice Use standardised outcome measures and evidence based practice in order to maximise therapy input and evaluate service delivery, and encourage others to do the same. To complete 6 week and 6 month reviews as per national guidelines as part of the multidisciplinary team. Ensures timely submission of required statistical information that accurately reflects his/her clinical activity. Supervises the clinical training of physiotherapy undergraduate students and facilitate their professional development. Provide effective supervision and teaching in order to support development of junior team members, assistants and students reporting back when there are issues to address. Participates in the Trusts appraisal scheme both as an appraiser and as an appraisee To assist in the organisation of in-service training and facilitation of this. To undertake any other duties which may be reasonably regarded as within the nature of the duties, responsibilities and grade of the post as defined, subject to the proviso that appropriate training is given and that wherever possible significant changes of a permanent nature shall be mutually agreed and incorporated into the job description in specific terms and the post re-evaluated if the change is likely to result in a job evaluation score change.
