Main duties of the job

As a Physical Health Care Assistant in our SMI Outreach Team, you will conduct comprehensive physical health assessments and deliver interventions for patients with Severe Mental Illness. Your duties include performing clinical procedures, visiting patients at home, and collaborating with healthcare professionals to ensure seamless care. You will also educate patients on health and well-being, helping them navigate local resources.

Key qualities include strong communication skills, a compassionate approach, IT proficiency, and the ability to work independently and as part of a team. Your work will directly contribute to improving patient outcomes and overall quality of life.

The post holder will be expected to attend and fully complete a nationally agreed induction programme and undertake an on-going training to achieve a minimum of NVQ2 / VRQ Good level of secondary education with demonstrable literacy and numeracy skills

Desirable

NVQ level 2 or 3 or other relevant qualifications

Further Training or Job Related Aptitude and Skills

Essential

Confident working alone. Holistic approach to care. Good time management skills. Good written communication skills. Able to prioritise work. IT literate with basic keyboard skills.

Experience

Essential

Previous experience working in a care or rehabilitation setting Experience of using Office computer programs including Excel, Word and Outlook

Desirable

Previous experience of managing own caseload of clients. Previous experience of clinical supervision.

Personal Qualities

Essential

Well-developed communication and interpersonal skills. To demonstrate English verbal and written communication skills. Able to work as a member of a team.

Ability to travel between sites across Buckinghamshire Know own limitations. Able to work alone with patients. Able to use initiative. Ability to work flexibly to the needs of the service.
