We are seeking a dedicated and compassionate Physical Health Care Assistant to join our SMI Physical Health Outreach Team at Oxford Health NHS Trust. This vital role focuses on improving the physical health of individuals with Severe Mental Illness (SMI) by performing health assessments and delivering targeted interventions.
Key Responsibilities- Conduct comprehensive physical health assessments and deliver interventions for patients with Severe Mental Illness.
- Perform clinical procedures, visit patients at home, and collaborate with healthcare professionals to ensure seamless care.
- Educate patients on health and well-being, helping them navigate local resources.
- Understanding of confidentiality.
- Knowledge of mental health conditions/illnesses.
- Good level of secondary education with demonstrable literacy and numeracy skills.
- Confident working alone, with a holistic approach to care.
- Good time management skills, written communication skills, and ability to prioritize work.
- IT literate with basic keyboard skills.
- Previous experience working in a care or rehabilitation setting.
- Experience of using Office computer programs, including Excel, Word, and Outlook.
