Join Bromley's integrated primary and community transformation team to be a key part of designing and delivering our whole-system model of care, in with One Bromley's strategic the wider NHS, Bromley faces growing demand for health services in the future; transformation and improvement is required to ensure the sustainability of primary care and the healthcare system as a whole.
We are seeking a transformation leader who will be able to work with all parts of the ICB, provider strategic groups and with partner organisations to initiate and deliver primary and community-based transformation projects which will improve health outcomes for patients and reduce dependence on hospital-based services. Your responsibilities will include leading service transformation, redesign and innovation, developing and leading integrated neighbourhood initiatives, facilitating collaborative working across organisational boundaries and engaging with clinicians, other health care staff, patients and wider stakeholders.
You will be responsible for leading the design and implementation of primary and community care improvement initiatives, engagement programmes with the public and frontline staff, reviewing services and pathways and developing close working relationships with GP practices and other One Bromley health and care partners. You will be responsible for affecting change which consider and balance provider and policy objectives in a complex landscape.
Main duties of the job
The postholder will be responsible for identifying quality and efficiency improvement opportunities, keeping patient outcomes and experience at the centre of your work. You will demonstrate an analytical, evidence-driven approach, conducting research, benchmarking, financial assessments and options analyses to present robust, persuasive business cases to senior leaders.
The post-holder will need to possess tenacity and be resilient in response to the challenges presented by the current context. The successful candidate will be able to demonstrate their knowledge, technical and analytical skills as well as excellent communication skills, both written and verbal, and an ability to build strong working relationships across the health and care system.
An understanding of pathway redesign, outpatient transformation and the primary care landscape will be important for this role. Experience in service redesign of one or more clinical areas such as diabetes, cancer, haematology, cardiology, respiratory, neurology or gynaecology would be beneficial but not essential.
