Practice Pharmacist

St Andrews, United Kingdom NHS Scotland Full time

NHS Fife has a unique opportunity for a pharmacist to join the team, working between GP practices and Community Hospitals within the North East Fife area. This is a new role, and the initial site for this role will be within St Andrews Community Hospital, where there are two GP practices, and two community wards. The post will have a flexible approach to time within each area, but candidates should expect approximately 60-80% of their time to be spent within GP practices, with 20-40% on wards. Going forward, there may be opportunity to extend this role to also providing services to the Adamson Hospital in Cupar, where there are also both GP practices and wards, with additional team support.

You will support GP practices to deliver work under the GMS contract including: medicines reconciliation; acute, repeat and serial prescription requests; cost efficiencies; and polypharmacy medication reviews. In addition, at ward level, this post will support medicines medication reviews (including realistic prescribing), medicines safety & governance and discharge planning. An additional aim of the post holder is to facilitate seamless transition of care between settings. There is an opportunity for full utilisation of an Independent Prescribing qualification on a daily basis.

Pharmacists, and the wider pharmacy team, are embedded as part of the multi-disciplinary team in all practices, to help deliver high quality pharmaceutical care, ensuring patients get the best possible outcomes from their medicines, avoiding harm and waste.

You will be supported by Senior Practice Pharmacists and will work with an expanding and developing team of Pharmacy Technicians and Medicines Management Support Workers across the NHS Fife Pharmacy and Medicines Directorate.

This role is an excellent developmental opportunity to give Pharmacists a breadth of experience across the diverse, interesting & engaging clinical nature of primary care, whilst combining a hospital role. You should be competent in time management and prioritisation.

Whilst experience of primary care would be desirable, prospective candidates currently working in other sectors are encouraged to apply as full training will be provided. Also, previous experience in hospital is desirable but not essential as full training will be provided.

As an integrated pharmacy service, we run regular staff development sessions on a range of clinical topics, and along with our MDT colleagues, there is regular opportunity to attend wider educational and CPD events.

Fife has something to offer everyone and for further details, please see the attached job pack.

For an informal discussion about the post, or to arrange to spend time with a member of the team to see the role, please contact Cara Mackenzie, Lead Clinical Pharmacist – Primary Care cara.mackenzie@nhs.scot or Anne Wilson, Lead Clinical Pharmacist – Community Health anne.wilson1@nhs.scot.

A requirement of this post is to become a member of the Protection of Vulnerable Groups (PVG) Scheme Prior to appointment.

NHS Scotland is reducing their full-time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week. If you have any questions or concerns, please contact the Recruiting Board.

NHS Fife is legally obliged to ensure all its employees are legally entitled to work in the United Kingdom. If you are not a United Kingdom (UK) or Irish National, you are required to confirm your right to work in your application.

NHS Scotland is committed to encouraging equality and diversity among our workforce and eliminating unlawful discrimination. The aim is for our workforce to be truly representative and for each employee to feel respected and able to give their best. To this end, NHS Scotland welcomes applications from all sections of society.
