About the Role

Holland House Surgery is seeking a highly skilled Clinical Pharmacist to join our multidisciplinary team. As a Clinical Pharmacist, you will play a vital role in delivering our medicines optimisation goals and providing high-quality patient care.

Key Responsibilities
  • Conduct face-to-face and remote medication reviews with patients to ensure safe and effective medication use.
  • Answer medication-related queries from patients and other clinicians, providing expert advice and guidance.
  • Optimise medicines to manage chronic diseases, improving patient outcomes and reducing healthcare costs.
  • Manage acute medication requests, ensuring timely and effective treatment.
  • Perform medicines reconciliation, identifying and resolving medication discrepancies.
  • Audit work to ensure compliance with regulatory requirements and best practices.
  • Liaise with our established PCN pharmacy team to share knowledge and best practices.
Person Specification

We are looking for a highly motivated and proactive Clinical Pharmacist with:

  • Professional registration with the General Pharmaceutical Council.
  • Commitment to fulfilling annual revalidation requirements.
  • Good clinical pharmacy knowledge and expertise.
  • Self-motivation and a proactive approach to work.
  • A prescribing qualification or commitment to working towards this.
