We are seeking a skilled Clinical Pharmacist to join our multidisciplinary team and contribute to our medicines optimisation goals.
Key Responsibilities
- Conduct comprehensive medication reviews with patients to ensure safe and effective medication use.
- Provide expert advice on medication-related queries from patients and healthcare professionals.
- Develop and implement strategies to optimise medicines management for chronic diseases.
- Manage acute medication requests and ensure timely resolution.
- Perform medicines reconciliation to prevent medication errors.
- Audit and evaluate medicines use to inform quality improvement initiatives.
- Liaise with our established PCN pharmacy team to ensure seamless collaboration.
Person Specification
- Professional registration with the General Pharmaceutical Council.
- Commitment to annual revalidation requirements.
- Excellent clinical pharmacy knowledge and skills.
- Self-motivated and proactive approach to work.
- Prescribing qualification or commitment to working towards this.
Desirable Qualifications and Experience
- Experience working in a GP Practice.
- Clinical Diploma or further post-graduate qualifications or similar experience.
- Familiarity with GP Clinical systems including EMIS Web.
