Clinical Pharmacist

This is an exciting opportunity to work in a dynamic and forward-thinking General Practice, combining patient-facing, analytical, and administrative elements of work.
About the RoleWe are seeking an additional Clinical Pharmacist to join our established pharmacy team to help deliver our medicines optimisation goals. The role will be challenging and varied, with a focus on conducting face-to-face and remote medication reviews with patients, answering medication-related queries, and optimising medicines to manage chronic diseases.
Key Responsibilities- Conduct medication reviews with patients to ensure safe and effective medication use
- Provide expert advice on medication-related queries from patients and other clinicians
- Optimise medicines to manage chronic diseases and improve patient outcomes
- Manage acute medication requests and ensure timely access to necessary medications
- Perform medicines reconciliation to ensure accurate and up-to-date medication lists
- Audit work to monitor and improve medication use and safety
- Liaise with our established PCN pharmacy team to ensure seamless communication and collaboration
We are looking for a highly motivated and proactive individual with a strong commitment to patient care and safety. The ideal candidate will have:
- Professional registration with the General Pharmaceutical Council
- Commitment to fulfilling annual revalidation requirements
- Good clinical pharmacy knowledge and skills
- Self-motivation and a proactive approach to work
- Prescribing qualification or commitment to working towards this
