About the Role
Holland House Surgery is seeking a skilled Clinical Pharmacist to join our pharmacy team and contribute to our medicines optimisation goals.
Key Responsibilities
- Conduct face-to-face and remote medication reviews with patients to ensure safe and effective medication use.
- Answer medication-related queries from patients and other clinicians to provide expert advice and support.
- Optimise medicines to manage chronic diseases and improve patient outcomes.
- Manage acute medication requests and ensure timely access to necessary medications.
- Perform medicines reconciliation to identify and resolve medication discrepancies.
- Audit work to monitor and improve medication use and safety.
- Liaise with our established PCN pharmacy team to share best practices and expertise.
Person Specification
We are looking for a motivated and proactive Clinical Pharmacist with:
- Professional registration with the General Pharmaceutical Council.
- Commitment to fulfilling annual revalidation requirements.
- Good clinical pharmacy knowledge and skills.
- Self-motivation and a proactive approach to work.
- A prescribing qualification or commitment to working towards this.
