Do you have a formal or informal background of education delivery, or supporting education delivery and have any counselling skills or experience in supporting the wellbeing needs of people?

If you are passionate about helping others, and interested in learning more about people who live with focused health care needs, then a new patient educator vacancy within the community heart failure service may be the just the right role for you ;

We are looking for a new Patient Educator who can:

Learn and deliver a focused Heart Failure education and information package to patients who are newly diagnosed with heart failure and are referred into the community specialist service.

Main duties of the job

Education delivery will include:

The post holder to work autonomously with patients, without direct supervision and support.

To deliver the approved and formatted heart failure education package initially on a one-to-one basis with patients, delivering in a variety of community settings with key carers and family present

Deliver education utilising virtual platform mechanisms.

To work closely and in collaboration with the clinical community heart failure specialist nursing team, GPs and practice nurses, internal colleagues within the partnership trust

To support HF education delivery sharing with other local partners / NHS trusts teams.

As heart failure education mechanisms are evaluated, innovative changes in service delivery are anticipated and future delivery could incorporate group HF education sessions

With the clinical specialist team to participate in Heart Failure education package delivery to GPs, practice nurses, internal colleagues and other NHS Trusts as part of this service, as well as reviewing opportunities to further develop specialist service education delivery.

The Patient Educator will work alongside the Heart Failure Specialist Nurses and the Cardiac Rehab team. They will take responsibility for the education of patients, empowering them to live a healthier lifestyle and to understand the implications of, and treatment paths for, their heart failure diagnosis.

Monitoring, recording and progressing patients education programmes in accordance with agreed guidelines, protocols and treatment regime.

Supporting the senior cardiology/heart failure staff to ensure that the educational component of the cardiac rehabilitation programme is clinically effective and high quality.

Preparing and delivering holistic health promotion and patient education for individuals attending cardiac rehabilitation and their carers

Providing advice to patients and carers in a variety of settings, both face to face and digitally.

Participate in local service audit, evaluation and research.

Person Specification

Qualifications & Training

Essential

GCSE or equivalent level including Maths & English NVQ level 4/diploma or equivalent relevant life science qualification or equivalent level of knowledge/experience

Experience

Experience of working within a specialised MDT

Desirable

Experience educating patients/clients Experience evaluating a patient/customer's knowledge base Experience of using digital content libraries Experience working in a project team

Skills, Knowledge & Abilities

Proficient in the use of Microsoft office: Word, Excel, Teams and Power-point Excellent communication skills Ability to prioritise workload in response to service needs Ability to work as part of the wider team Ability to maintain confidentiality and deal with situations in a sensitive manner Ability to apply evaluative and reflective techniques to personal practice, events and activities Trained/willingness to be trained as a key trainer in Health Literacy Awareness of when to escalate patient concerns to the clinical team Basic counselling skills

Insight into health inequalities across the local NHS networks Knowledge and experience of using the Trust's electronic record systems Knowledge and understanding of the Heart Failure treatment path or equivalent NHS pathway experience
