To assist in coordinating and supporting delivery of therapeutic interventions to long term health condition referrals across Teesside To assist in the development of partnerships and pathways with physical health care providers To monitor referrals within this patient group waiting for therapy and to liaise with the Leadership team and others to ensure timely transitions from referral to treatment. To work with a caseload primarily with clients from this patient group. To coordinate and support the development of a range of psychoeducational groups for various long-term health conditions.

To oversee the delivery of various psychoeducational groups delivered in a continuous rotation, ensuring appropriate facilitation, training, and allocation. To liaise with staff within the wider team to ensure effective management of the referral to treatment process. To provide support and guidance to staff within Primary Care and other teams about the mental health needs of clients with long term health conditions To develop best practice guidelines for working with this client group and to provide training, support, and guidance to staff within Impact on Teesside Clinical Teams. 2.

To work alongside members of the Primary Care Core NHS Talking Therapies team To create and maintain effective working relationships within the PCPWP Team and elsewhere. To offer line management to qualified PWPs working with this client group To offer advice and support to other members of the wider Primary Care Team, including GPs and other Health Professionals, on mental health issues, and in relation to appropriate referral pathways for patients with mental health needs. To liaise effectively with Secondary Care and voluntary sector organisations and services To attend meetings and provide reports as required. To assist in the development and promotion of the service 3.

To provide a range of therapeutic interventions To provide assessment, including risk assessment, of clients referred for psychological therapy within the Primary Care setting. To select and deliver a range of appropriate therapeutic interventions in collaboration with the client (for example, CBT, and psychoeducational groups) To work within an appropriate time framework. To make appropriate onward referrals following assessment for clients requiring other services, including statutory and voluntary agencies To maintain clients confidentiality within agreed guidelines 4. To provide a professional and ethical service To maintain appropriate records including all required assessment and evaluation tools To undertake relevant Continuing Professional Development activities both as required for the purposes of continued professional accreditation or registration and as identified through Impacts Professional Development and Appraisal System To undertake an appropriate amount of clinical supervision from a qualified Clinical Supervisor within Impacts Supervision Programme To operate within agreed NHS, service and professional guidelines and Codes of Practice To ensure a safe and secure working environment for self and others and to act as necessary within Health and Safety and other guidelines.
