SUMMARY OF POST DESCRIPTION

Working under the direct supervision of the Nurse Practitioners/Doctors/Practice Nurses and strictly in accordance with specific practice guidelines and protocols, the Health Care Assistant will assist the practice clinical team in the provision and delivery of prescribed programmes of patient care.

ROLE AND RESPONSIBLILITES


The prime role and responsibility of the HCA is to ensure that Brook Medical Centre is fit for purpose to enable the clinicians to practice in a safe tidy and clean environment.


CLINICAL PRACTICE
The post holder will support the nursing team to achieve the following

  • Implementation of treatment programmes that promote health and wellbeing.
  • Implementation of individual treatment plans for patients with a long term condition.
  • Implementation of treatment plans for patients at risk of developing a long term condition as appropriate.
  • Work with other health care professionals to support treatment of long term conditions, in accordance with management plans and in line with national and local policies and practice needs.

DAILY DUTIES AND RESPONSIBILITIES

  • New Patient Health Checks
  • ECG recording
  • Blood Pressure Monitoring (BP)
  • Phlebotomy
  • Spirometry
  • Removal of Sutures
  • Immunisations and Vaccinations under PSD (Annual Training required)
  • Flu/pneumonia injections

  • Processing of laboratory samples

  • Preparing and maintaining environments and equipment before, during and after patient care.
  • Assisting in the assessment and surveillance of patients health and wellbeing i.e.
  • Completing work up template prior to long term condition (LTC) clinics with Nurse Practitioner/Practice Nurse
  • Assisting in hypertension clinic with Nurse Practitioner/Practice Nurse
  • Assisting GPs/NPs during the performance of minor surgery.
  • Helping raise awareness of health and wellbeing and how it can be promoted
  • Call & recall patients on Long term registers:
  • Diabetic, CHD, Hypothyroid, Mental Health, Dementia, Epilepsy, COPD, Depression, Cancer, AF, PAD, Stroke.
  • Assisting with the maintenance/calibration of all clinical equipment and keeping an equipment log
  • Order consumables and assist with record keeping of drug stocks
  • Other responsibilities based on competencies and training frameworks within a normal healthcare assistant role.

DELIVERING A QUALITY SERVICE
The post holder will work under the guidance of the Nursing Lead to

  • Recognise and work within the professional code of conduct of the Practice
  • Produce accurate, concise documentation of patients records consistent with legislation, policies and procedures
  • Participate in the maintenance of quality governance systems and processes across the practice
  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality
  • Deliver care in accordance with guidelines and evidence based care
  • Collaborate on improving the quality of health care in partnership with other clinical teams, responding to local and national policies and initiatives

DEVELOPMENT
The post holder will under the supervision of the Nursing team and will

  • Participate in planning and implementing changes within the areas of care and responsibility, within the remit of the HCA role in accordance with guidelines, protocols and policies
  • Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning that is within the remit of the HCA role

COMMUNICATION


The Practice has a multi-disciplinary team that the post holder will have to liaise with effectively on a daily basis.

The post holder is expected to represent the practice in a courteous and professional manner at all times. The post holder is often the initial staff member seen by patients
- e.g. new patient health checks or work up clinics and must be able to

  • Demonstrate effective communication skills to ensure patients are fully informed, of systems and services
  • Anticipate barriers to communication and take action to improve communication at different levels, cultural backgrounds and preferred ways of communicating
  • Act as an advocate when representing patients and colleagues
