Our new Community Support Worker will join our Home First Team in Kerrier delivering reablement as part of a collaboration between community, acute and social care providers.

The reablement approach supports people to return to or remain in their homes. We are looking for candidates with the ability to motivate individuals in order to aid their recovery and maximise their independence, achieve their goals and get home from hospital quicker.

You will be part of a dedicated team and will be provided with bespoke training to support your development in this role. You will need to demonstrate good communication skills and a positive approach to older people who may have communication difficulties or who may have challenging and complex health needs including frailty.

The team provide a 7 days a week service to community patients and we are happy to discuss flexibility around hours and days worked.

Main duties of the job

The main purpose for this post is to provide a versatile and responsive service to ensure that opportunities for patients' reablement are optimised within the community setting

The Community Support workers within each locality will form a part of an Integrated Care Pathway for older patients that involve a range of services to prevent hospital admission and support discharge from hospital

The post holder will be expected to deliver packages of care to patients in their own home; this may also include Residential homes and nursing homes and we offer a mileage allowance for journeys in your own car and access to pool cars where availability allows.

The post holder will be expected to be flexible in working patterns to meet the needs of the service and will be rostered to provide the required service however, we are happy to discuss flexible opportunities with you.

Just over 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.
