Job summary

Our Hep C service is looking for a motivated and enthusiastic full-time (2 yr FTC) Band 6 CNS to join our specialist team.

The post holder will work closely with the CNS team, ID Consultant and members of the team. They will be required to develop and deliver an outreach service to North Yorkshire with the use of the outreach van. The role will require daily travel driving the outreach van to the North Yorkshire area, and occasional work within Hull and East Riding.

Working as part of the service you will use your specialist knowledge to provide support, guidance, care and treatment to patients with Hep C.

The post holder will ensure that they provide patient-focused service at all times. Due to distance and the large scope of the North Yorkshire area, flexible working hours will be necessary to ensure that patients will be seen at suitable times for their needs. This requires long days, early morning and evening work.

We are seeking someone who is passionate about making a difference to people living with Hepatitis C and assisting with the acceptance of a new diagnosis and treatment

You will need to possess clinical assessment skills, excellent communication and interpersonal skills.

You will often be the first point of contact for patients and other medical teams and will play a key role as a member of the multi-disciplinary team.

You will be based within a team but you must be able to work independently and autonomously.

A valid UK driving license is essential.

Main duties of the job

The post holder will be an experienced nurse, who acts within their professional boundaries. They will join our existing hepatitis specialist nurse team, which manages patients with viral hepatitis. Many patients will be from areas of deprivation and disadvantaged groups that are a population that is often difficult to engage.

The team requires expansion to meet the growing demands of increased

testing and diagnosis and referrals of hepatitis C, and subsequently those

requiring treatment.

Strong partnership working across the complex pathways for patients is essential to ensure patients have access to both clinical expertise and local delivery of care.

Under the direction of the Lead Clinical Nurse Specialist, the post holder will be responsible for the work-up and treatment of patients with Hepatitis C. The majority of work will be based on the Hepatitis C outreach van with a support worker within the North Yorkshire area. Travelling time from the office base in Castle Hill Hospital to and from areas of North Yorkshire is included within the working hours and accommodated by shift patterns.

The post holder will also be required to assist in delivering staff education programmes, ongoing advice, and clinical supervision to other health care professionals. When training is complete, the post-holder will work unsupervised and manage their own clinics, with regular monitoring/clinical supervision from the Lead Clinical Nurse Specialist.
