Our ideal candidate will be able to provide support, education andproblem-solving for the primary health care team. Main duties of the job
To work with partners & existinggroups to develop the referral pathways for support. Draw on and increase thestrengths and capacities of local communities, enabling local VoluntaryCommunity and Social Enterprise organisations (VCSE) and community groups toreceive social prescribing referrals. Keep up to date with organisations & feedback to team.
To work with individual clients,particularly around poor mental wellbeing & low level mental health issues,1:1 basis to build trusting relationships to ensure they are accessing theright support, time & place.
Provide personalised support toindividuals, their families & carers to take control of their mental health & wellbeing, live independently & improve their health outcomes, as a keymember of the PCN multi-disciplinary team. Develop trusting relationships bygiving people time and focus on what matters to me. Possess genuine listeningskills and provide short-term interventions such as CBT, and solution-focusedtechniques.
To manage and prioritise your owncaseload, in accordance with the needs, priorities and any urgent supportrequired by individuals.
Alongside other members of the PCNmulti-disciplinary team, work collaboratively with all local partners tocontribute towards supporting the local VCSE organisations and community groupsto become sustainable and that community assets are nurtured, through sharing intelligence regarding any gaps/problems identified in local provision withcommissioners & local authorities

Social prescribing empowers people to take control oftheir health and wellbeing through referral to link workers who give time,focus on what matters to me and take a holistic approach to an individualshealth and wellbeing, connecting people to community groups and statutoryservices for practical and emotional support. Link workers also supportexisting groups to be accessible and sustainable and help people to start newcommunity groups, working collaboratively with all local partners. Socialprescribing can help PCNs to strengthen community and personal resilience andreduces health and wellbeing inequalities by addressing the wider determinantsof health, such as debt, poor housing and physical inactivity, by increasingpeoples active involvement with their local communities. It particularly worksfor people with long term conditions (including support for mental health),people who are lonely or isolated, or have complex social needs which affecttheir wellbeing.
The Mental Wellbeing SPLW will support patientsparticularly around their mental wellbeing and low level mental health, stillworking with the whole person to develop a practical and emotional supportnetwork.
The post holder will liaise with a variety ofpartners, community and voluntary organisations across the geographical patchand the wider programme. Excellent communication skills and local knowledge areessential in order to engage and work with members of the public. Willingnessto undertake mandatory and role specific training within a specified timescalewill also be essential.
The main role of the MentalWellbeing Social Prescribing Link Worker (SPLW) will be to work within our practice and Primary Care Network (PCN), to support strengthening community and personal resilience andreducing health and wellbeing inequalities by addressing the wider determinantsof health, particularly those impacting mental health / wellbeing, such asdebt, poor housing and physical inactivity, by increasing peoples activeinvolvement with their local communities. The post holder will particularlywork with people with poor mental wellbeing and low level mental health issues.

Toapply asset based community development, engagement and consultation techniquesas appropriate to engage the target groups/individuals.
To develop new services / activities for the community based onidentified need particularly around poor mentalwellbeing and low level mental health issues.
Provide short-term interventions such as CBT, and solution-focusedtechniques.
Toensure appropriate information and resources are available for staff andmembers of the public, in order for individuals to make the correct choices toaid them with changes to their behaviour that have a positive impact on theirhealth.
