Job summary
Halton OOH District Nursing service, are seeking a committed and enthusiastic individual to join their team as a community staff nurse, covering the evening service.
The post is hours per week over 3 evenings, hours are 17:00 -
Applicants must have excellent interpersonal and communication skills, have a Patient centred approach to care, be highly motivated and able to work autonomously as well as part of a team.
The district nursing service provides nursing care to the housebound population of Halton to patients in their own homes as well as residential care homes. We also provide a treatment room service for ambulant patients and provide a comprehensive range of clinical skills.
You will work under the direction of the District Nursing Sister delivering:
Administration of IV medication/Medicines management Health Promotion End of life / supportive care. Holistic assessments Patient centered Care. Complex wound management Catheter care.
Main duties of the job
The successful candidate will perform holisticassessments of patient nursing needs. Plan, assess, implement and evaluate caredelivery according to changing health needs utilising prescribing skills.
Ensure practice is evidence based throughundertaking audit and critically appraising literature to achieve a highstandard of care.
Liaise with the District Nurse team leader toensure information sharing in relation to patient care / changes to care planand the continuity of care and reviewing of complex care packages.
Mentoring, Teaching and Assessing.
Liaises with all health care professionals andother agencies to ensure effective service delivery.
Involve patients and carers in the planning anddelivery of care.
Maintain accurate records in accordance with NMCStandards.
To participate in the health needs assessment ofthe practice population to identify areas of unmet need.
Ability to work without direct supervision and tosolve problems.
Work on a rota basis to facilitate weekend workingacross the district nursing teams

To undertake chronic disease managementreviews using appropriate template.
To work without direct supervision.

Skills & Attributes
Essential
Able to perform complex assessments, plan, implement and evaluate care packages. Wide range of clinical skills including: Male & Female catheterisation. Experience of working within palliative care. Able to communicate sensitive/unpleasant information to patients, relatives and/or carers. Knowledge of professional and NHS issues relating to specialist knowledge. Able to demonstrate initiative. Ability to develop effective interpersonal relationships with colleagues across health and social care settings Ability to communicate at a high level both verbally and written. Good IT skills

V100 - V150 nurse prescriber Teaching and Assessing qualification Audit and research. Supra pubic catheterisation. Tissue viability qualification. Leg Ulcer management qualification. IV Therapy Chronic disease management. Experience of working within palliative care.
