Job Description

We are seeking a highly skilled and experienced Community Staff Nurse to join our District Nursing team at Bridgewater Community Healthcare NHS Foundation Trust. The successful candidate will work closely with patients, families, and other healthcare professionals to provide high-quality patient care in the community.

The post holder will be responsible for undertaking comprehensive assessments of patient nursing needs, planning and implementing programmes of care, and evaluating their effectiveness. They will also support the team leader in coordinating complex and continuing care packages and maintain accurate records in accordance with NMC standards.

Responsibilities
  • Perform comprehensive assessments of patient nursing needs as the named nurse
  • Plan, assess, and evaluate programmes of care according to changing health needs under the direction of the District Nurse
  • Support the team leader in coordinating complex and continuing care packages
  • Maintain accurate records in accordance with NMC standards
Requirements

To be successful in this role, you will need:

  • A qualified RN First level/RGN qualification and NMC registration
  • Previous community experience and/or post-registration nursing experience
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
