Our Urgent Community Response Support Workers form an essential and well-respected part of our healthcare teams across a number of dynamic settings; out in the community, in our acute and community hospitals.

You will be a valued member of the team through our comprehensive training and induction programme and our commitment to learning and development will satisfy even the most ambitious person

The team – The Urgent Community Response Support Worker (UCR SW) will work as part of the multidisciplinary team providing support with personal care with patients in their own home or care setting.
The post holder will contribute to the assessment of care needs and the delivery of planned care and provide advice and information to patients, relatives and carers under delegation and direction of the registered clinician.
The post holder will practice in accordance with Trust standards and policies and to work within the boundaries of their role and individual competence.

Urgent Community Response is a joint Health and Social Care Service providing support to people in their own homes in times of crisis, to avoid unnecessary admissions to hospital and facilitate discharges home from hospital.

The service provides care over 7 days a week days a year, with shift patterns covering the hours from : hours until : hours. The service provides care across Sidmouth, Seaton, Axminster and outlying areas and you will be part of a team providing a quality service promoting independence, dignity and support. This service aims to avoid unplanned hospital admission for people who could safely be maintained and supported at home, as well as enabling people to return to their home as quickly as possible from hospital.

Previous community experience is desired but not essential as the teams will give you the support and guidance you need to excel. You will join a dynamic multi-disciplinary team, working with Health and Social Care Teams to respond to urgent situations, to enable people to stay in their own homes or ensure a timely discharge from Acute and Community Hospitals.

You will be provided with a comprehensive induction programme which will include support to complete the National Care Certificate.

Successful candidates will have a Level 3 Diploma / NVQ 3 in Health and Social Care or equivalent or have equivalent experience of working with older people in a community setting. You will also need to be competent with IT as electronic recording is an essential part of the role. As this is a community based position, a driving license is essential as is access to a vehicle.

Urgent Community Response Support Workers are lone workers within the community, although support is always available.

Working Pattern:  We care for our patients around the clock so we will need you to work a range of shift patterns. These range from, Early shifts between : hours to : hours and Late shifts from : hours to : hours and at least alternate weekends. Please do check this at your interview.

Depending on your shift pattern and working hours, you may also receive additional enhancements to your pay.
