This is an exciting opportunity to join the Sidmouth, Axminster and Seaton (SAS) cluster (part of the Royal Devon University Healthcare NHS Foundation Trust’s Community Care Group) as Interim Urgent Community Response (UCR) Team Manager.

We are looking for a Registered Nurse for this role.

Working in close partnership with the Professional Practice Team, Devon County Council and teams within Royal Devon University Healthcare (RDUH), the post-holder will be responsible for the leadership and delivery of Urgent Care Response services within the Cluster through the operational management of NHS employed Nurses and Support Workers. Therapists work in the team on a rotational basis but are line-managed by the Therapy Department in the SAS Cluster.

The postholder will provide clinical leadership and support to the team regarding patient treatment and management. They will provide highly specialist assessment, diagnosis, treatment education and advice to patients and their carers. The specialist caseload will include a wide range of acute and chronic cases, many having complex presentations. The postholder will work in conjunction with the Community Service Manager to ensure effective day to day operational management of the UCR service for the cluster and will plan, coordinate, deliver and evaluate the UCR service for patients in the local community. The postholder will ensure professional practice excellence through the provision of line management, supervision, casework monitoring, consultation and practice development which focuses on promoting excellence in both assessment, treatment and the attainment of care planning outcomes.

We link closely with our colleagues in the Community Nursing and Community Rehab teams to deliver a cluster and locality-based service. The successful candidate is also expected to contribute to the 7 day service between the hours of 8am - 8pm on a rota basis. 

You will require wide-ranging clinical skills and community experience to be able to work autonomously, as well as part of a team. You will need excellent communication skills, adaptability & resilience to change. Lone working is required at times
