We are looking for a Registered Nurse or Paramedic for this role.

The Urgent Community Response Team is a Multidisciplinary team consisting of Nurses, Support Workers, Coordinators, an Adult Social Care Assessor, a Pharmacist and Therapists. Working in close partnership with the the wider Multi-disciplinary team this role focuses on keeping patients in the most appropriate location for care, and promotes admission avoidance and early supported discharges. The post-holder will be responsible for the assessing patients with a variety of needs, sometimes within a 2 hour response window. They will also be responsible for the ongoing care and management of those patients.

The post holder will provide clinical assessments of patients and be responsible for their ongoing clinical and holistic treatment and management. They will provide highly specialist assessment, diagnosis, treatment education and advice to patients and their carers. The specialist caseload will include a wide range of acute and chronic cases, many having complex presentations. The post holder will work alongside a team of Associate Community Matrons across the cluster providing assessments and managing patient's ongoing health and care needs.

We link closely with our colleagues in the Community Nursing and Community Rehab teams to deliver a cluster and locality-based service. You will require wide-ranging clinical skills and community experience to be able to work autonomously, as well as part of a team. You will need excellent communication skills, adaptability and resilience to change. Lone working is required at times

The Associate Community Matron will:

Provide detailed clinical and holistic assessments of patients in the community Will often be working remotely without direct supervision Provide supervising, teaching and involvement with the development of peers, and other relevant team members, carers and patients Develop expert assessment, and clinical skills relating to complex medical conditions and case management. Provide care and treatment as close to home as possible involving making close links with secondary care providers to facilitate coordination, as appropriate, through the patient pathway Contribute to development and the local delivery of fully integrated health and social care urgent care response services Provide oversight and management of patients on the existing caseload and be responsible for organising their ongoing care solutions Be responsible for managing any patient safety issues arising on the patients on the caseload Work within their professional boundaries to professional practice standards Manage and organise staff to deliver the highest quality urgent care services within the resources available to meet needs

Working Pattern: hours per week. The successful candidate is also expected to contribute to the 7 day service between the hours of 8: and : on a rota basis.
