The Urgent Community Response Team is a Multidisciplinary team consisting of Nurses, Support Workers, Coordinators, an Adult Social Care Assessor, a Pharmacist and Therapists. Working in close partnership with the the wider Multi-disciplinary team this role focuses on keeping patients in the most appropriate location for care, and promotes admission avoidance and early supported discharges.

The post-holder will be responsible for the assessing patients with a variety of needs, sometimes within a 2 hour response window. They will also be responsible for the ongoing care and management of those patients. The post holder will provide clinical assessments of patients and be responsible for their ongoing clinical and holistic treatment and management. They will provide highly specialist assessment, diagnosis, treatment education and advice to patients and their carers.

The specialist caseload will include a wide range of acute and chronic cases, many having complex presentations. The post holder will work alongside a team of Associate Community Matrons across the cluster providing assessments and managing patient's ongoing health and care needs. We link closely with our colleagues in the Community Nursing and Community Rehab teams to deliver a cluster and locality-based service. You will require wide-ranging clinical skills and community experience to be able to work autonomously, as well as part of a team.
