About the Role

The Royal Devon and Exeter NHS Foundation Trust is seeking an experienced Registered Nurse to join our Seaton Community Nursing team as a Clinical Care Specialist in Home Nursing.

As a Clinical Care Specialist, you will be responsible for providing high-quality nursing care to patients in their own homes, working closely with our Multi-Disciplinary Teams, including the Community Matron and Our Seaton Hospice at Home team.

You will aim to maintain patients in their preferred place of care, enabling them to maximise their independence and optimise their quality of life.

The ideal candidate will have excellent communication skills, a patient-centred approach, and the ability to work collaboratively as part of a team.

Job Responsibilities

  • To assess, provide and review nursing care, advice and information to patients and carers within the home environment or alternative care setting.
  • To work under the direction of the Nurse Specialist (Community) and the Community Nurse Team Manager, using your skills and knowledge as a Registered Nurse.
  • To hold a designated caseload and deputise in the absence of the Nurse Specialist (Community).

Requirements

To be successful in this role, you will need:

  • A valid NMC registration as a Registered Nurse.
  • Experience of working in a community nursing setting.
  • Excellent communication and interpersonal skills.
  • A patient-centred approach and ability to work collaboratively as part of a team.
