Job overview

·The post holder will be responsible for assisting the peritoneal dialysis team, primarily at Sydenham satellite unit and in the community.

·When required the post holder will be expected to work at Denmark Hill site, based on the needs of the service.

·Primary roles will include working as part of a team to effectively deliver PD services to a wide community.

·The post holder will be assisting with setting patients PD machines up daily in their own homes, for the patient to connect in the evening, this will include working weekends and the post holder will be required to drive to patient’s home address. 

·The post holder will be expected to follow algorithms to implement changes to patient care as appropriate; this will include basic measurements (weight and blood pressure etc). 

·The post holder will be expected to liaise with qualified members of staff when making these changes. 

·The post holder will also be required to carry out assessments based on protocols and algorithms regarding suitability for PD.

The post holder will be required to assist the qualified community PD team with daily workload, prioritising as required

Main duties of the job

·Perform assessments of fluid balance and implement changes to patients care based on a pre-approved algorithm.

·Work collaboratively with; patients, families, other health care professionals and external agencies relating to Peritoneal Dialysis.

·Organise and prioritise workload, using time management skills to plan for the day.

·The post holder will be required to take laboratory samples (Bloods, PD effluent and swabs (training will be provided)

·Collect, collate and accurately maintain accurate complete and comprehensive patient records.

·The post holder is expected to be flexible with their work base between: KCH, Sydenham Dialysis unit and the community.

·The post holder will be expected to assist with PD home assessments. To assess the suitability of a patients environment for peritoneal dialysis following a pre-defined algorithm. (Appropriate training will be provided).

·Delivering fluid and ancillaries in an urgent/emergency situation. Manual handling skills will be required.

·The post holder is expected to drive between community visits (a car will be provided for use in work hours only) this will include motorway driving.
