Job summary

*Jointly responsible for co-ordinating the work of the team with the TOC Professional Lead and Adults Intermediate Care Team (AICT) Leads influencing clinical decisions and monitoring the quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to own field of practice underpinned by theory and experience on a rota basis.

*Take a joint lead role in collaborating with AICT leads in delivering effective working relationships with key health and social care partners across integrated care pathways and from urgent care into the Integrated Locality Teams, community units and PRRT.

*To support the delivery of performance targets within the PSEH (Portsmouth South East Hampshire) System.

*To support the development of a culture of service improvement which promotes quality and safety.

*Actively contribute to team development on a routine basis.

Main duties of the job

*Jointly responsible for the organisational delivery of and contribution to the development of the Transfer of Care Team, and its key role in the PSEH System.

*To support the Head Of Service in managing the performance of the discharge pathway from acute care into Community Services.

*Take a lead role in delivering effective working relationships with key health and social care partners across the integrated discharge pathway.

*Line management responsibility for staff within assigned groups.

*Development of expert clinical practice to include working with team to develop protocols, according to national and local guidelines for safe and effective delivery of the services.

*Use your expert knowledge in determining the best care pathway, and therefore the best health and wellbeing outcome for patients in the care of the Portsmouth system.

*Contribute to effective discharge planning and/or referral processes to health care providers and support agencies to meet identified health needs.

*Ensure that the outcomes and performance of designated services are appropriately monitored and audited in the clinical setting and that learning is shared throughout the organisation.

*Provide a role model in the use of research based clinical evidence, through working alongside colleagues sharing knowledge, attitude, and clinical expertise, and by monitoring the effectiveness of care at local level.

*Work with staff to ensure efficient organisation and use of clinical and manpower resources.

About us

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by the end of 2024.

Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.

The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.

Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on the day the trust is formed. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.
