Role Summary

Specialist Community Occupational Therapist, South Molton and Chulmleigh Health and Social Care Team.

Provide specialist assessment, diagnosis, treatment and advice to patients and their carers. The caseload will include a wide range of acute and chronic care cases, many having complex disabilities and needs. Some services will also include terminal care cases. Be part of a multidisciplinary team working in a community setting.  Ensuring that therapy input is integrated into the patients’ overall care plan. Work as an autonomous practitioner working without direct supervision and at times lone working. Provide supervision, training and support to junior staff and students. Work with managers to develop the service in line with patient need and trust wide developments, and to help provide an equitable service across the Trust. Participate in weekend working.

Are you an experienced, enthusiastic and passionate Occupational Therapist looking for an opportunity to work with a team of community based Occupational Therapists, Physiotherapists, Occupational Therapy Apprentice, Assistant Practitioners, Trainee Assistant Practitioner and Therapy Support Workers within an integrated multidisciplinary health and social care team?

We have the exciting opportunity to appoint a Specialist Occupational Therapist to join our enthusiastic community-based therapy team in South Molton. The role of Specialist Community Occupational Therapist encompasses clinical, teaching, leadership and research functions.

We work with adults registered under South Molton and Wallingbrook (Chulmleigh and Winkleigh) medical centres and cover a rural area close to Exmoor in beautiful North Devon. The community therapy team has twelve members including occupational therapists, physiotherapists, an occupational therapy apprentice, assistant and trainee assistant practitioner and clinical support workers. Our integrated health and social care team includes the Community Therapy team, Community Nursing, Community Matron, Rehabilitation Nurse and Adult Social Care Team. We work closely with social prescribers in the GP practices for links with and inclusion of the people we work with into community activities. Typically, the day starts with members of the team triaging new referrals and a team meeting to prioritise and plan the day. We positively encourage joint working between members of the community therapy and broader health and social care team.

Our work is consistently varied and stimulating, and the role offers scope to work with people with acuity and with those with long term and complex health and social conditions. As a team we support:

Unplanned urgent work including supporting discharge and short term services including urgent care reviews. Rehabilitation and habilitation in a range of community settings through medical, social and public health approaches.

The post offers a flexible and supportive opportunity for personal and professional development. The community therapy team works with people with a wide range of disabilities and acute or long-term conditions striving to deliver excellent patient centred care. As a Community Occupational Therapist, you will be managing a clinical caseload of patients who vary in their level of complexity and who have a range of health conditions. The occupational Therapy team includes a B7 Advanced Practitioner Occupational Therapist, a B6 integrated Health and Social Care Occupational Therapist, two B6 Community Occupational Therapy posts including the post advertised and an Occupational Therapy Apprentice. The team work flexibly leading to scope to realise a range of personal and professional development opportunities.

We host occupational and physiotherapy students from Plymouth University and the University of the West of England. We have regular peer group meetings, team members support delivering learning in our assistant/trainee assistant practitioner and support worker peer group sessions. The Trust is active in research and quality improvement programs. We hold regular team meetings and often invite external facilitators to support team reflection and learning. All team members are invited to and encouraged to take leadership in support to one another in 1:1, peer and team settings; in audit and service development.

Pay: The salary advertised for this position reflects the Agenda for Change 4-5 rates, inclusive of a pay award implemented from October 4. If you join before this date, the pay award will be backdated to April 4 or your start date, whichever is later. Until then, remuneration will be based on the 3-4 pay bandings (Pay scales for 3/ | NHS Employers).
