Job summary

A newly developed, exciting opportunity to advance into a Band 5 Tobacco Dependency Specialist for the Peri-operative team is being advertised by SWFT. With fundamental supervision and support, you will obtain the highest level of training and development to advance your expertise as well as your skills.

As the initial point of contact for patients who have been identified as smokers, the Tobacco Dependency Specialist role will involve promptly reviewing the patients' needs and providing high-quality, evidence-based behavioural support in accordance with the National Centre for Smoking Cessation and Training Standard Treatment Programme and NICE Guidance. This would involve assistance and recommendations for accessing medication, such as Behavioural Therapy and Nicotine Replacement Therapy (NRT), working under the direction and supervision of the Nurse Lead for the service and the agreed Trust protocol.

The role will also consist of ensuring specialist Tobacco Dependency information, advice and guidance is accurate and current, and at all times supporting the design, implementation and maintaining the Tobacco Dependency service, systems and processes required to ensure that the Tobacco Dependency service can be accessed appropriately and that internal and external standards for recording and monitoring activity, including outcomes achieved, are met.

Main duties of the job

As part of the NICE Guidance and The NHS Long Term Plan SWFT have created a specialised team to help treat Tobacco addictions within a hospital setting. Theteam offer specialised support to people who are admitted to hospital who smokeand/or use Tobacco, to help them quit the Tobacco addiction and to be Tobaccofree during an inpatient admission (temporary abstinence).

The Tobacco Dependency Service recognises smoking and the use of Tobacco as an addiction and that it is a chronic relapsing clinical condition that prematurelykills at least half of the people who smoke. Smoking is seen as a medical conditionthat can be treated rather than it being a lifestyle choice. This post will tackleinequalities in health through specialised treatments and support for people whowant to stop smoking.

The post holder will require a NMC pin as a registered nurse who will take asupportive role in strengthening contribution to patients within this speciality with a specific focus to Elective Surgical Patients, who have agreed to quit prior to their planned surgeries. The post holder responsible for delivering Tobacco Dependency treatment through holistic assessments, Pharmacotherapy and Behavioural Therapy. Furthermore, to deliver programmes of care, collect NHSE mandatory data and establish Tobacco Dependency clinics within outpatients in line with The NHS Long Term project.

Expertise & Excellence in Clinical Practice

An understanding and experience working within outpatient and/or asimilar setting, this must include establishing and/or delivering specificclinics.

Ability to assess, plan, implement, deliver and monitor tobaccodependency advice and support sessions throughout their outpatientjourney. This must include advice on Nicotine Replacement Therapymedications, on-going care management, and working withinrecommended protocols and guidelines provided. This may involveindividual, telephonic, on-line and group work sessions in a variety ofsettings and venues.

A sound understanding of developing individually tailored support plansincluding recommending treatments based on assessed needs.

Be able to provide evidence based general health, lifestyle advice andsupport to people using the service healthy life style choices,physical activity, and stress management. This will include individualand group-based interventions, signposting and referring to otherhealth, community, and voluntary services.

Have experience in and be competent to participate in educatingpatients about the benefits of stopping smoking, particularly prior tosurgery, and give support to those wanting to quit. Promote smoke freehomes and support and advice about passive smoking.

Undertake nursing interventions consistent with evidence-basedpractice, transferring and applying knowledge and skills to meetpatients needs.

Maintain accurate up to date and comprehensive records at all times.Furthermore, to contribute to the monitoring and evaluation of theTobacco Dependency service.

Ability to have difficult but motivational conversations with patientsabout the risks of smoking, including the importance of a smoke-freehousehold.

Clinical

Take clinical responsibility for patients within your allocated area ofwork.

Continuously assess, plan, implement and evaluate treatment, usingappropriate programs/methods of care and to ensure a high delivery ofquality care in accordance with Trust policies, procedures andguidelines.

Undertake relevant training to maintain the skills and knowledge asnecessary through participation in continuous development. TheNational Centre for Smoking Cessation Training (NCSCT) practitionertraining is essential to this role.

Respond effectively to referrals within stated timescales self-referralsfrom the individuals, and those from colleagues /other workers/GPs.

Ensure that the relevant equipment is provided and maintained on anagreed schedule. Report any failings of equipment or otherenvironmental factors to the Tobacco Dependency Nurse SpecialistLead.

To be involved in the monitoring and evaluation of client feedback,offering recommendations as part of the team to improve services.

Be aware of how to report incidents, concerns or complaints andescalate any concerns or complexities to the Tobacco DependencySpecialist Nurse.

Alert the appropriate line manager to any issues in work (includingconcerns about individuals or work in the community) and maintainconfidentiality.
