Job summary

Are you a compassionate Registered Nurse RN seeking a new opportunity. We have Community Staff Nurse positions available within the team, based at Bridgwater.

Our service covers the urban and rural areas of Bridgwater Bay. These positions are dayshifts couple of twilight shift over a month and one in four weekends which include additional unsocial hours enhanced payments. There are no night shifts. We offer flexible working solutions where possible to ensure a work life balance and part time hours will be considered.

You'll need to be a car driver with your own vehicle to visit patients. We offer a generous mileage allowance to cover the use of your car for NHS business, which includes both mileage and depreciation.

Main duties of the job

Community Nurses work as part of a team providing high quality and compassionate care to individuals. They are autonomous practitioners who undertake a holistic assessment and write a personalised care plan following the assessment for individual patients in their own homes. They are responsible for safe delegation, evaluation of the care plan and discharging patients. They also have responsibility as a named nurse for individual patients. Teaching and supporting Assistant Practitioners, HCAs, students, carers and patients, liaising directly with all other Health and Social Care providers to ensure that patients needs are met

You will develop a broad range of nursing skills, including IV medication administration, palliative and end of life care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management.

You will need to make autonomous clinical decisions and create holistic care plans, with the support of your colleagues and Team Leaders. Comprehensive training and guidance will be provided by our two dedicated Clinical Lead nurses.

Bridgwater District Nurses are split in 6 teams, 5 based out of Bridgwater Hospital and 1 from GP surgery in Nether Stowey.

Job responsibilities

The band 5 staff nurses are autonomous practitioners who undertake a holistic assessment and write a personalised care plan following the assessment for individual patients in their own homes. They are responsible for safe delegation, evaluation of the care plan and discharging patients. They also have responsibility for the following.

Acting as a named nurse for individual patients Teaching and supervising Assistant Practitioners, HCAs, students, carers and patients Liaising directly with all other Health and Social Care providers to ensure that patients needs are met Manage caseload in absence of the band 6 Team Leader Have awareness of budgetary requirements to include the management of Equipment and resources within agreed budget parameters Organise their workload ensuring that visits are planned according to priority and location Take an active part in team meetings providing feedback on patients care and Contribute to the multi-disciplinary planning of individual patient care Documenting all care given in accordance with Trust policy Communicating effectively with all members of the district nursing team and wider multi-agency primary care team Having an understanding of the roles of different agents within the team to ensure efficient communication that optimises patient care Respecting the diversity of individual patients and colleagues. Ensuring that maintenance of their dignity and respect for them is an integrated part of all activities undertaken This role requires a developing knowledge of community nursing in the broadest sense and exceptional interpersonal and communication skills to support patients with a wide range of conditions. Main clinical duties includethe following.

Assessment, Care Planning, Review, Discharge and Referral. First assessment and care planning for clinical need, continence, End of Life Care, Holistic assessment, Care planning review which may include discharge, MUST assessments, Waterlow assessments Falls assessments Frailty assessment Fast Track assessments CHC checklist assessment, MCA and best interest assessment, Advocacy, Safeguarding referral, Continence & Catheterisation. Undertaking catheter care, Female urethral catheterisation ,Male urethral Catheterisation Supra pubic catheterisation. Bladder scan, assisting with continence assessments. Digital removal of faeces, Trans anal irrigation, First catheterisation for acute retention of urine, Undertaking a trial without catheter, Assessing bladder and bowel dysfunction and prescribing products, Medicines Management ,Administration of Eye Drops, Administration of Ear Drops, Administration of skin care products, Administration of Suppositories, Administration of Enema, Administration of Bladder Maintenance Solution, Administration of Pessaries, Administration Low Molecular Weight Heparin,Job Description for Band 5 Staff Nurse 3, Administration of Insulin, Administration of Hydroxocobalamin, Administration of Denosumab, Administration of Just in Case Medicines, Administration of S C Medicines through a Syringe Pump, Administration of Red Drugs, Administration of IV drugs via a pump, Application of skin patch medicines, Administration of Percutaneous endoscopically placed gastrostomy PEG medicines, Administration of Flu vaccine PGD, Administration of Shingles vaccine PGD, Administration of oral medicines, Administration of other SC or IM medicines, Wound Management, Pressure area checks and review of MUST, Waterlow & equipment, Skin tear redressing, Post op wound care Grade 1 to 4 pressure ulcers, Removal of sutures and clips, Packing of wounds, Fungating wounds, Doppler assessment, Arterial leg ulcer, Multilayer and short stretch bandaging, Negative Pressure Wound Therapy, Larvae Therapy, Complex Wounds, Chest Drains, Additional Skills, Care of central lines and peripherally inserted central catheters, Care of Portacath, Removal of chemo pump, Venous Cannulation, Verification of expected death in adults, Non Invasive Ventilation, Venepuncture and cross match, Physiological observations, ECG, Equipment Assessment.
