Job summary

As lead practice for the Gorleston Primary Care Network The Millwood Partnership is looking for a PCN Care Co-ordinator to support Gorleston PCN in delivering the PCN contract. This is a supportive role promoting health processes and intitiatives and helping to ensure that patients are able to access them. The role will develop as the contract changes from year to year.

Main duties of the job

Youwill need outstanding people skills, a caring and compassionate nature, be anexcellent communicator and have a desire to learn in this ever changingenvironment

This is anexciting new role with scope to develop and deliver service improvements,working with the GP practices and wider community health teams.

Reception cover for our PCN colleagues will also be included in this job role.

About us

The Millwood Partnership is a forward thinking practice in Bradwell close to the east coast of Norfolk. It is lead practice for Gorleston Primary Care Network, with the Clinical Director and Management Lead both part of the practice team.

Clinically well-staffed we have 3 partners and 3 salaried GPs to cater for a diverse population of around 20,000 patients. The practice is led by the Partners and team of three managers, with a support staff including nurse practitioners, practice nurses, HCAs, pharmacists, paramedic, administration, prescription clerks and receptionists. We are lead practice for our Primary Care Network and are a teaching and training practice to medical students and GP Trainees respectively.

Job responsibilities

Key responsibilities

Undertake work in line with PCN directedpriorities including reception duties at Falkland for your PCN colleagues

Proactively identify and work with a cohort ofpeople to support their personalised care requirements

Ensure regular and consistent communication withthe referrer regarding patient progress and any complications or guidance

Raise awareness of health promotion, screeningand NHS health checks in practices

Support the evaluation of treatment programmes thatpromote health and well-being

Co-ordinate clinics, liaising with theschedulers and contacting the identified patients with appointments

Manage patient initiated calls forhelp/signposting etc., ensuring patients are directed to appropriate service

Document and monitor aspects of patientco-ordination and service delivery, supporting data collection and audit usingthe practices clinical system

Demonstrate the ability to recognise and respondappropriately when faced with a sudden deterioration or emergency situation,alerting the team or enabling a rapid response

Support national screening and immunisationprogrammes and encourage uptake

Monitor referrals to ensure tasks are completedand care delivered by keeping in regular telephone contact

Work closely with and refer to PCN socialprescribing link workers were a patient is identified as potentiallybenefitting from this service

Person Specification

Qualifications

Essential

Diploma/ HNC level/ NVQ Level 2 in a health or social care related subject, or Relevant experience in a similar role Demonstrable commitment to professional and personal development

Desirable

Training in motivational coaching and interviewing or equivalent

Experience

Essential

Experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field (including unpaid work) Experience of data collection and providing monitoring information to assess the impact of services Experience of partnership/collaborative working and of building relationships across a variety of organisations Experience of working with or in general practice Working in a multi-disciplinary setting where influence and negotiation is required Knowledge/familiarity with medical terminology Experience of dealing with patients or the general public

Desirable

Experience of supporting people, their families and carers in a related role (including unpaid work) Experience in use of databases Vulnerable adults awareness Experience of care of the elderly
