The Macmillan Palliative and End of Life Care (PEOLC) Transformation Lead hosted by Royal Trinity Hospice will be primarily responsible for supporting the implementation of the new adult community specialist palliative care model across the ICS. The post holder will bring together statutory and non-statutory partners, including the charitable hospice sector, across health and care to embed local priorities to bring about quality improvement in community based specialist palliative care for the local population. The post holder will support the statutory core responsibility of the Integrated Care System (ICS) to ensure that system wide accessible high quality, safe palliative, and end of life care, tailored to the needs of the individual (Health and Care act 2022) is available.

  Main duties
  • Lead on the implementation of the new model of care for adult (18+) community-based specialist palliative care services across NWL, addressing the eight key issues and developing the five enablers transformational work for successful delivery.
  • Facilitate coordination and collaboration among healthcare stakeholders for an integrated approach to delivering the ‘enablers’ which underpin the new model of care – addressing inequalities, common approaches to training, digital and information management, and workforce development
  • Develop a programmatic approach to the work required across NWL to deliver on the changes agreed in the new model of care and ensure clear ownership and timelines for change.

North West London Integrated Care Board’s (NWL ICB) vision is to ensure all NW London residents and their family/ carers/ those important to them have equal access to high quality palliative and end of life care and support for adults (18+), that is sustainable and coordinated, and which from diagnosis through to bereavement reflects their individual needs and preferences. To achieve this vision, the programme includes the following work streams:

  • Support more people to receive care and die in their place of choice
  • Improve equity and equality of access to Palliative and end of life services for all NWL residents
  • Improve outcomes for patients and their families/carers/those important to them who are accessing our palliative and end of life care services
  • Mitigate health inequalities in relation to services and meeting needs for NWL residents

Education and Qualifications

Essential

  • Educated to Masters level or equivalent level of experience working at a senior level, supported by a project management qualification for example PRINCE2, Agile or Managing Successful Programmes (MSP).

OR

  • Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience.

Desirable

  • Registered healthcare professional with relevant professional registration (such as nurse, occupational therapist etc.) or a manager with relevant healthcare experience.

  Experience

Essential

  • Experience of cross organisational or system wide transformation.
  • Substantial experience of working in the PEOLC specialism.
  • Experience of leading, managing and delivering significant change projects involving stakeholders.

Skills

Essential

  • Project and programme management skills and experience.
  • Well-developed interpersonal and communication skills
  • Present complex, sensitive, or contentious information with senior stakeholders
