Job summary

To effectively assess, plan and provide specialist care and support to people and their relatives/carers affected by life limiting illnesses, and to patients and their relatives/carers who are at the end of their lives. To participate as part of a multi-disciplinary team, working collaboratively with colleagues, patients, carers, and other health and social care professionals to ensure optimum patient management. To work within statutory legislation and local policies and procedures in relation to palliative care. To work autonomously and manage a shared case load. To participate in providing a 7 day face to face service. To act as a role model to other nurses and support the delivery of palliative care.education within the Trust. To act as a resource and advocate for patients and their relatives/carers. To participate in audit/research.

Main duties of the job

To acts as a resource of clinical excellence in the speciality of palliative care nursing, and ensure that national and local policies are implemented and followed. To assess, plan, implement and evaluate holistic patient care based on sound rationale and evidence-based practice to influence and implement treatment To participate in the management and triaging of new referrals and taking part in daily allocation. Maintain and be responsible for electronic and written records in line with the organisation. To offer skilled emotional and psychosocial support and refer on to other members of the multidisciplinary team as appropriate. To participate in the weekend face to face on call rota. To ensure symptoms are optimally managed and anticipatory prescribing needs are identified to provide for imminent changes in condition. To recognise and take appropriate action in issues related to MCA and Safeguarding. To develop and maintain close working relationships with all statutory and/or voluntary agencies involved in patient's care. To promote evidence based practice with measurable standards and outcomes through the process of audit and benchmarking. To adherence to governance processes through a thorough understanding of local guidelines and policies. To participates actively in the delivery of education on Palliative and end of life care Trust wide.

Job responsibilities

Responsibilities to patients:

In collaboration with other services and the multidisciplinary teams to deliver a high standard of care for the benefit of patients, their relatives /carers and the public. To assess, identify and explore complex emotional issues which patients and families face as a result of living with life limiting disease and when approaching end of life. To be an educative resource for patients and relatives/carers. To recognise ethical dilemmas and act as an advocate for patients, their relatives/carers.

Responsibilities to policy and service development:

To ensures palliative care policies are updated in response to national and local changes to practice. To work collaboratively with external providers and agencies to align with regional and national initiatives.

Responsibility for resources:

To ensures that value for money and cost effectiveness is a consideration in practice and in developing schemes for mentoring and support.

Responsibility for leading and managing:

To manage the caseload in an effective, efficient and timely manner. To act in a professional and supportive manner toward junior staff and students. To be a role model and lead by example in supporting excellent communication and care. To support the management of weekly and weekend rotas.

Professional development

To actively participate in own professional development, adhering to professional governing bodies and governance structures, including re-registration requirements. Acts as a resource in the development of others through education. Person Specification

Qualifications and Training

Essential

MSc level learning RN / RM - Part 1 Relevant specialist course or similar - palliative care Mentorship course supervising and assessing) Advanced training/skills in invasive/therapeutic/diagnostic procedures relevant to speciality - palliative care In-depth knowledge of anatomy related to speciality Knowledge of conditions, management and treatment - Long Term Conditions including but not exclusively cancer, COPD, MND, dementia

Desirable

Knowledge of quality standards Counselling course Non-Medical Prescribing Advanced Communication Skills Training

Experience

Essential

Senior clinical experience within their speciality - palliative and end of life care Experience in Practice Guideline Development Audit and Research Experience Evidence of clinical leadership qualities Ability to prioritise and organise workload Ability to work autonomously and as a member of a multi-professional team Evidence of using primary and secondary healthcare teams and resources Ability to clinically lead and direct staff Experience in patient advocacy
