Main duties of the job

The role will be for a Social Value Co-ordinator. You will be key in supporting patients to tackle the issues underlying their ill health, money issues, health issues, social issues and much more.

You will also support proactive management of frailty, to enable them to live healthy, independent lives for longer and to avoid hospital admission.

You will work closely with allied community health and social care workers, GPs and community groups to support service integration work around mental health, social care, community nursing and the voluntary sector.

As well as working closely with Therapists, ACPs, Complex Care Nurses, GPs Primary Care, Health and Social care co-ordinators, Well- being centre staff and other services, within our new ageing well clinic
