Job summary

A fantastic opportunity has arisen on Ward 35 for an enthusiastic pro-active person to work as a Discharge Support Assistant Band 3, we will consider Full-time or Part-time with flexible working hours.

We are looking for someone who is able to work on their own initiative as well as part of the great team. The Discharge Support Assistant's role is to improve outcomes for patients being discharged from the ward.

Early identification of potential needs and additional support is very important in achieving a short length of stay and reduces risk of potential readmission.

The ideal candidate will have a good understanding of the discharge process and be aware of some of the challenges in relation to facilitating a good discharge, have relevant previous hospital experience and a desire to progress in health care.

The candidate must have excellent communication skills, be confident in communicating with the multi disciplinary team and also patients and their families.

You will have or be willing to work towards completing training that enhances knowledge and skills, to undertake clinical tasks to improve assessment and diagnostics including cannulation. Have a flexible approach to meet the needs of the service and its users.

Main duties of the job

The post holder will provide a comprehensive service to the Clinical Management Group with a focus to facilitate effective and timely discharge of patients no longer requiring acute care.

The role requires a consistent and coordinated approach to the operational management of patient discharges, with an emphasis on maintaining timely and well planned discharge process as well as supporting the medical team to progress patient plans.

It is the intention that this post will contribute to reduced length of stay and/or contribute to a reduced risk of admission/re-admission for our patients, whilst maintaining safe and effective quality of care.

The post holder is required to work collaboratively with the multi-disciplinary team, internal and external to the organisation, potentially working between two or more clinical ward areas.

Job description

Job responsibilities

To focus directly on the needs of the patients requiring support and facilitation to be discharged from the acute hospital.

To develop close working relationships with ward teams, providing clinical and administrative support to the multi-professional team, having delegated responsibility under the direction of an appropriate registered healthcare professional.

Support the delivery of effective healthcare, relating to the discharge process and/or assisting with processes/procedures to enhance patient flow as identified by the multiprofessional team.

Undertake specified and supervised aspects of care and therapeutic intervention as delegated by the registered healthcare professional within an agreed protocol, sphere of responsibility and after completing appropriate competency training agreed by the trust.

To provide next day telephone follow-up as appropriate for patients discharged and take actions and/or escalate issues as required.

To undertake some clinical tasks relevant to the specialty in order to improve assessment and onward transfer of patients for procedures and/or diagnostics cannulation.

The post holder will also be expected to complete specialist training that enhances knowledge and skills to plan and prioritise effective discharges and identify appropriate external provider provision.
