Havering Liberty PCN is led by our ClinicalDirector and PCN Manager who work together with the GP leads from eachpractice. The PCN also works closelywith and is supported by its GP Federation, Havering Health Ltd, who are theemploying body for the workforce of the network. The PCN Clinical Pharmacistresponsibilities include but are not limited to the following:

To be aprescriber, or completing training to become prescribers and work with andalongside the general practice team

Toundertake thecare management of patients with chronic diseases and to undertake clinicalmedication reviews to proactively manage people with complex polypharmacy,especially older people, people in care homes, those with multipleco-morbidities (in particular frailty, COPD and asthma) and people withlearning disabilities or autism (through STOMP Stop Over Medicaoin Programme)

Provideexpertise in the use of medicines and person-centred medicines optimisationacross the network, including taking a central role in the clinical aspects ofshared care protocols and liaison with specialist pharmacists to reduceinappropriate use of high-risk medicines

Supportthe integration of general practice with wider healthcare teams (includingcommunity and hospital pharmacy) to help improve patient outcomes, reducewaste, and promote self-care, provide better access to healthcare, and managegeneral practice workload.

ManagingSafety and CAS alerts

Assistingwith QOF and incentive scheme audits, PINCER safety indicators

AttendingMedicines Management Prescribing forums quarterly and assisting practices inachieving Prescribing incentive Scheme targets.
