Job Description

Job Title: Clinical Pharmacist - Medication Review Specialist

Job Summary:

We are seeking a highly skilled Clinical Pharmacist to join our team as a Medication Review Specialist. The successful candidate will be responsible for undertaking clinical medication reviews for patients with chronic diseases and proactively managing people with complex polypharmacy, especially the elderly, people in care homes, and those with multiple long-term conditions.

Key Responsibilities:

  • Undertake clinical medication reviews for patients with chronic diseases
  • Proactively manage people with complex polypharmacy, especially the elderly, people in care homes, and those with multiple long-term conditions
  • Provide expertise in the use of medicines and person-centred medicines optimisation across the network
  • Support the integration of general practice with wider healthcare teams to improve patient outcomes, reduce waste, and promote self-care
  • Manage safety and CAS alerts, assist with QOF and incentive scheme audits, and manage availability queries
  • Attend Medicines Management Prescribing forums quarterly and assist practices in achieving Prescribing Incentive Scheme targets
  • Ensure all prescribing protocols are up to date and provide care home support, including annual meds reviews of patients
  • Discharge summary reconciliation, polypharmacy medication reviews, minor ailment clinic, and medication review clinic for respiratory, diabetes, cardiovascular, and colostomy products
  • Supervise and train reception staff in issuing repeat prescriptions and queries, and supervise and train new prescribing staff, including trainees and nurse prescribers
  • Attend Clinical and PCN meetings as required and provide telephone advice to patients for queries on medication and minor illness
  • Review repeat prescription policies and support staff in implementation

Requirements:

  • Registered with the General Pharmaceutical Council as a Pharmacist in the UK
  • Postgraduate Diploma or equivalent (or be working towards one) - desirable
  • Independent prescribing qualification (or to work towards this in the future) - desirable
  • A minimum of 2 years post-registration clinical experience
  • Understanding of working effectively within multidisciplinary groups
  • Demonstrable experience in clinical pharmacy practice covering a broad range of specialities
  • Recognises the roles of other colleagues within the organisation and their role to patient care
  • Demonstrates use of appropriate communication to gain the cooperation of relevant stakeholders
