Job summary

The role

The Advanced Critical Care Practitioner (ACCP) will function on the medical rota to provide care to critically ill patients in Cardiothoracic Critical Care. This is an advanced role, and the post holder will provide enhanced continuity and a high standard of management for patients on critical care and to deteriorating patients within Oxford Heart Centre.

The ACCP will perform clinical activities, some of which were previously in the domain of doctors and utilise a high level of clinical judgement and decision making. To function at this level requires the authorisation of the employer and successful completion of the two-year ACCP clinical training programme. The MSc in ACCP (or equivalent) is also required to meet Oxford University Hospital criteria for banding at 8a. The scope of practice and training for ACCP's is defined by the Faculty of Intensive Care Medicine (FICM), and it is expected that the postholder will register as a FICM member.

The ACCP is accountable for their own professional actions, working autonomously as a specialist within Critical ACCP must acknowledge any limitations in their knowledge and skills, acting within the formal code of conduct of their present statutory regulator.

Main duties of the job

Clinical practice

Participates in the medical rota system to provide Cardiothoracic Critical Care cover, supported by resident medically qualified trainees/fellows, and24/7 non-resident consultant cover

Ensures effective, timely review and management of an individual caseload of patients within the Cardiothoracic Critical Care Unit and for any patients referred within the Oxford Heart Centre

Works competently in the role of ACCP, being aware of the boundaries of the role, referring patients appropriately to other healthcare professionals as required

Clinical practice

Perform history taking, physiological and holistic assessment of critically ill patients regardless of location and respond appropriately

Requestlaboratory and radiological tests as required

Monitors patients compliance against treatment plans through interpretation of information from test results and clinical findings

Utilise critical care expertise and clinical decision-making skills to inform clinical reasoning and appropriate action, in the best interests of the patient

Prescribe safely and effectively, acknowledging own limitations and scope of practice as a non-medical prescriber

Authorise blood products as indicated by clinical situation and relevant diagnostic tests

Undertakes advanced airway management skills, including intubation when relevant assessments have been completed

Initiates and subsequently manages mechanical ventilation to ensure optimal ventilation

Assists with tracheostomy management in the critical care units and ward areas, including managing tube changes and de-cannulation

Assess and retrieve patients requiring critical care from other areas within the hospital

Initiate admission to, and discharge from the critical care units after discussion with the consultant in charge of the unit

Undertakes invasive procedures within the scope of practise such as insertion of arterial line, central line, peripherally inserted central catheters, chest drains

Recognise and take appropriate actions with reference to complications using appropriate referral system

Lead and respond to emergency cardiothoracic situations utilising expert clinical judgement and decision making

Undertake intra- and inter-hospital transfer of the critically ill patient according to agreed protocols as deemed appropriate

Participate in MDT meetings, ward rounds and case presentations which contribute to the decision-making process in the management of the cardiothoracic critical care patient

Communicates effectively with patients, their relatives and carers, colleagues and students

Leadership, Communication and Teamwork

Communicates well with staff, personalising their communication approach to ensure teams are conversant with relevant information

Excellent verbal, written and electronic communication skills so that professional advice or care guidance and instructions are well understood by patients, their relatives and/or carers, and members of the multi-disciplinary team

To liaise and work closely within the clinical / multidisciplinary team to ensure good working relationships are in place which maximise benefits to patient care

Actively engages in undertaking audit, research and service evaluation, using this to inform clinical effectiveness, clinical excellence and best use of resources to develop practice

To present findings of research, audit and service evaluation as requiredto identify areas of success or requiring improvement within the department

Promotes and support the application of research and evidence-based practice

To participate in the delivery of training programmes to hospital staff as appropriate

Education and Development

Attendance at local, regional and national educational events

Participate in teaching to medical personnel and the multi-disciplinary team within the directorate, and across critical care areas

Engages in regular evaluation of teaching activity and act on feedback to ensure delivery is appropriate to service needs

Undertake Continuing Professional Development (CPD) as defined by their current regulator to maintain and update professional competenceand maintain a professional portfolio

Participate in annual appraisal with assessment of clinical skills as recommended by FICM

To monitor the effectiveness and contribute to the development of the ACCP role

Participate in recruitment, training and supervision of ACCPs
