Job summary

We have an exciting opportunity for an enthusiastic and proactive registered pharmacist to join our team delivering innovative pharmacy services to the community.

No two days will be the same; you will have patient contact; you will work as part of a multi-disciplinary team; you will problem solve and you will have the opportunity to shape the pharmacy service you deliver.

Prospective candidates should be efficient, enthusiastic and confident to work autonomously within a supportive team and their sphere of competence. They should have excellent communication skills as the job will involve liaising with social care, clinical and community teams as well as patients and their carers.

You will have a desire to improve patient outcomes; an aspiration to participate in research, the will to learn and develop and the opportunity to influence the future of pharmacy services.

You will have a Masters in Pharmacy, or equivalent, independent non-medical prescribing qualification (ideally) and be registered with the GPC.

You will need to be flexible in your approach to work and able to travel as the role involves hybrid working.

Experience in community services is not essential but would be helpful.

Part time hours will be considered for suitable candidates.

If you are not already an independent pharmacist prescriber, you will be supported to gain this qualification. You will be offered a band 7 development post, progressing to band 8a upon completion of the qualification and performance review.

Main duties of the job

You will be working as part of a multidisciplinary team supporting our four primary care home teams deliver patient centered care primarily. You will also have the opportunity to provide clinical input into our bedded rehabilitiation and specialist palliative care unit.

To provide and oversee a Clinical Pharmacy Service to the four ECCH Primary Care Home Teams and beds with care focusing on delivering safe, evidence based costeffective and high quality pharmaceutical aspects of the Primary Care Home Team Service.

To support the provision of clinical pharmacy services to the community hospital ward and specialist palliative care beds.

To ensure that there is compliance with medicines legislation; national and local guidance and to manage risks associated with medicines use.

To ensure appropriate governance is in place regarding medicines, including leading on the response to any drug safety alerts or changes in clinical guidance and escalate as appropriate.

To provide professional expertise, advice, support and training on medicines management policies and medicines optimisation to health professionals and directly manage the PCH clinical pharmacy technicians.

To organise and participate in regular training and feedback sessions as appropriate

To deputise where appropriate (and agreed) for the Head of Pharmacy and Medicines Optimisation

Job description

Job responsibilities

To develop and provide a high qualityClinical Pharmacy Service to ECCH Primary Care Home Teams (PCH) including individualpatient medicines review, counselling, medicines reconciliation and fullclinical screening of prescriptions to ensure safe and cost effective use ofevidence based and rational use of medicines, in line with local policies andnational guidance ensuring medicines are optimised and risks to patients areminimised.

Tosupport the provision of clinical pharmacy services to the community hospitalward and specialist palliative care beds. Participate in ward rounds and MDTs where appropriate.

To provide professional expertise inrelation to drugs or medicines to PCH staff; patients and carers and othersincluding medical practitioners.

To support and implement efficientmedicines management initiatives to ensure the safe and secure handling ofmedicines; reduce drug waste and manage drug expenditure.

To prescribe within sphere ofcompetence and where appropriate the necessary medicines to meet the needs ofthe patient.

To participate in MDTs and PCHhuddles where appropriate.

To work closely with PCH pharmacytechnicians to enable patients in our care to be empowered to manage theirmedicines.

To facilitate the medicines elementof discharge to enable safe and efficient discharges.

To participate in and undertakeaudits as agreed including collecting and recording data for KPIs.

To support the wider pharmacy andECCH teams in delivering safe patient centred care through implementation ofthe Pharmacy and Medicines Optimisation strategy including providing professionaladvice; training and education; developing and reviewing relevant policies andSOPs.

Tomanage PCH based pharmacy technicians.

Experience

Essential

Significant post-registration experience in acute hospital, community health services or community pharmacy To analyse, interpret and evaluate specialist medicines related information or data, using clinical and professional judgement. This includes interpreting reports from any pharmacy computer system about drug usage and expenditure, clinical intervention reports, data from clinical trials and published papers, and changes in medicines legislation and policy (local and national). This information could inform decisions about an individual patients drug treatment ( data from the pathology system) or protocol & guideline development To interpret prescriptions for drug problems such as drug dosage, drug interactions and appropriateness of therapy. Considers legality, clarity and risk associated with prescribed medicines and prescriptions and makes interventions to ensure safety and optimal therapy Experience in planning, developing and delivering pharmacy services Experience of being responsible and accountable for own professional actions, working independently within professional and organisational boundaries and being responsible for the actions of others within their area of responsibility

Desirable

To communicate sensitive and highly complex specialist medicines related information and advice to patients & carers, consultants and their medical teams, and other healthcare professionals. Types of sensitive and specialist medicines and pharmaceutical information include: choice of therapy (including formulary issues), choice of formulation and route of administration, side effects, adverse drug reactions, use of drugs where its action and effects may be influenced by patient disease state, biochemical parameters, liver and kidney function, diet, gender, ethnicity, pregnancy, breast feeding, age related problems. This could be in sensitive situations including cancer treatment, terminal illness and the effect of drug administration or prescribing errors or other drug misadventures.
