This is an exciting opportunity to join the existing Medicines Management team at New Hayesbank Surgery. We are a member of Mid Kent PCN comprising of 7 practices. This role will be collaborating with colleagues but main workplace will be New Hayesbank Surgery.

The successful candidate will work as part of a mult-disciplinary team in a patient facing role, taking responsibility for areas o chronic disease management and clinical medication reviews.

Ideally, the successful candidate will have a prescribing qualification.

The post is funded via the Additional Roles and Reimbursement scheme and the post holder will be enrolled on the CPPE pathway.

Overview of Organization

New Hayesbank Surgery is a forward thinking and innovative surgery based in the centre of Ashford Kent.

We are registered with the CQC and proud of our "outstanding" rating. We have a large multi-disciplinary team who support the GP partners in providing the best possible care for our patients. We are also the Lead Practice for our PCN and one of our GP Partners is Clinical Director.

We are a training practice and also run a 7 day a week ophthalmology service which includes cataracts, wet AMD and general ophthalmology. We also have a number of in-house clinics.

We have a successful and innovative Medicines Management Team and the successful candidate will form a part of this team.

Job Purpose

Provideexpertise in clinical medicines review and address public health and socialneeds of patients in GP practices

Reduceinappropriate poly-pharmacy and wasteful prescribing through clinicalmedication review

Reconcilemedicines following hospital discharge and work with patients and communitypharmacists to ensure patients receive the medicines they need post discharge

Contributeto reductions in medicine related hospital admissions and readmissions bysupporting patients to get the best outcomes from their medicines andidentifying and addressing medicines related issues.

Interfacewith community and hospital pharmacy colleagues and develop referral processesbetween primary care professionals including the promotion of the repeatdispensing service

Managethe repeat prescribing reauthorisation process by reviewing requests for repeatprescriptions and medicines reaching review dates

Managepatients and health care professionals medicine queries

Increasequality and safety of prescribing through mechanisms such as audit and PDSAcycles

Managepractice formularies to improve the quality, safety and cost effectiveness ofprescribing

Implementdrug withdrawals and alerts MHRA aimed at improving medicines safety

Workwith primary care professional and patients to implement NICE and otherevidence based guidelines

Runclinics where medicines are the main interventions anticoagulantmonitoring clinics

Contributeto multimorbidity reviews and multidisciplinary reviews

Providemedicines information and training to Practice healthcare professionals andadmin staff

Reviewdaily pathology results for patients on known medicines.

Actas a source of medicines information for all of the practice team and patients( around doses, side effects, adverse events, possible alternatives around out of stocks)

Undertakeminor ailments triage; dealing with minor ailments and triaging patietnsappropriately.

Takepart in Virtual Ward and other MDT meetings

Supportclinicians with care home prescribing/MARS charts

CORERESPONSIBILITIES

Towork within the practice based team to undertake medication reviewsparticularly in high risk groups such as:

- Frail elderly

- Poly-pharmacy

- Substance misuse

- Patients on high risk medicines

- STOPP/START identified patients

- Revolving door Hospital admissions

Improve prescribingpractice through educational support for all prescribers within the practice

Lead on where changesin evidence require changes in prescribing across patient population wherea drug is withdrawn or indications change

Liaise withcolleagues in community pharmacy to align support for medicines adherence suchas MUR and NMS

Support improvementsin clinical care through practice based audit and implementing change

Prescribing advice toprescribers in practice temporary non availability of drugs

Ensure patient safetywhen they are transferred between care providers through reconciliation ofprescribed medicines

Qualifications& Training for this Job

NB:it is anticipated that the level of qualification held may vary according tothe level of position and the components of the role being carried out

Completion of anundergraduate degree in Pharmacy, and registration with the GeneralPharmaceutical Council

Minimum of 2 yearspost graduate experience in pharmacy, as demonstrated within a practiceportfolio

Member of the RoyalPharmaceutical Society (RPS)

Member of or workingtowards (through foundation) RPS Faculty membership

May hold or beworking towards a prescribing qualification

May hold or beworking towards a postgraduate pharmacy qualification
