The Hospital at Home Teams within Fife provide a service for the frail elderly population (predominantly over 65yrs), which prevents acute hospital admission or facilitates early supported discharge where acute hospital admission has taken place. It allows people to have a period of “acute” medical care within their own home, which will in turn allow the person to recover from a period of illness without needing hospital admission.

We are seeking a Nurse Practitioner to join our well-established Hospital at Home service within Adamson Hospital in Cupar. You will be required to deliver a high standard of safe and effective acute medical patient care within patient’s homes and must have the ability to work autonomously in this evolving service.

You should ideally have a clinical decision-making qualification at degree level or above. This should include the ability to work autonomously as a Nurse Practitioner as well as having clinical skills including venepuncture, ECG, IV additives and cannulation. Excellent communication and time management skills are essential in this role. A non-medical prescribing qualification would be desirable but not essential; however, it would be a requirement to undertake this in the future when courses are available.

Individuals who already hold the necessary qualification in clinical decision making would commence on Band 6. Those applicants who are interested in developing into the role following the successful completion of academic qualifications will commence on Band 5 in line with Annex 21(pay and banding of trainees). 

Candidates who commence on band 5 will be required to successfully complete the appropriate qualification/s or training as outlined within the Annex 21 competency framework prior to progressing to band 6. Trainees are paid a percentage of the pay band maximum for the qualified rate which is dependent on the duration of the training period (see Terms and Conditions). 

Candidates who meet the full requirements of the person specification for band 6 will commence payment at band 6 with no requirement to progress through the competency framework.

The ability to travel within the area in a timely manner is essential to respond to deteriorating patients and provide daily routine clinical interventions.

This post involves flexibility to work over a 7 day period, working shifts covering the period from 08.00 – 22.00 Cross team working is an expectation of the role to support absence and vacancies.

A requirement of this post is to become a member of the Protection of Vulnerable Groups (PVG) Scheme Prior to appointment.

NHS Scotland is reducing their full-time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
