Lead Nurse- Pathway homeless team

Job summary
The post holder will be responsible for the enhanced hospital management of homeless people referred to the team or identified by the team.
The post holder will work in partnership with other members of their own multi-disciplinary team, as well as other hospital teams and outside agencies in order to achieve successful discharges. They will also provide specialist discharge advice and support to other health professionals in the hospital as necessary.
The post holder will work across wards, in A&E, and may also outreach into the community to support patients post discharge.
The dual aims of the Pathway Homeless Team are to improve the quality of care for homeless patients, whilst reducing potential delayed and also premature discharges. There is an overarching aim to reduce future unscheduled admissions and A&E attendances.
Main duties of the job
The post holder will be a homeless health specialist in their own right, and will be able to provide advice to other members of the team in identified specific areas. In addition to providing in-depth knowledge of the homeless health services that can be referred to on discharge.
You will be engaged in significant partnership working aimed at improving hospital discharge for individuals, and also for groups of patients where patterns emerge, this will be with NHS, Housing, Social Care and Voluntary Sector organisations.
The post holder will provide direct support to medical & surgical teams, therapists & the Ward Sisters in discharging their patients in a safe and timely manner.
