About the Role:

The Assessment Coordinator is a critical position at the Single Homeless Project, responsible for facilitating young people's access to the youth accommodation pathway in Greenwich. This role involves triage and intake, as well as supporting young people in navigating their journey towards independence.

**Key Responsibilities:**

  • Provide specialist assessment and interventions for a designated caseload.
  • Mediate where needed and advise/facilitate restorative work with the young person and their family.
  • Navigate between and across specialties to create a multi-agency support plan.
  • Work within systems and families to enable young people to return home.

Requirements:

  • Two years' experience of assessing the needs of young people in a residential/accommodation setting.
  • Proven experience and skills in delivering casework and planned support for young people.
  • A strong understanding and practical knowledge relating to issues that typically disrupt progress in journeys towards independence, such as mental health, substance use, self-harm, domestic violence, and anger management.
  • Evidence of ability to navigate complex systems and create effective support plans.
