Ackerman Pierce Ltd is seeking an experienced Homeless Prevention Team Lead to join our growing team. As a key member of our Street Homeless Team, you will play a vital role in rehousing individuals who have experienced rough sleeping and providing them with sustainable housing options.

Main Responsibilities:

  • Manage and oversee a team of outreach workers to ensure goals are being met and performance is monitored.
  • Locate, verify, and engage with individuals found to be rough sleeping or bedding down in the area, providing support to improve their outcomes.
  • Develop personalized housing plans to support clients throughout their transition into permanent accommodation.
  • Build trust-based relationships with clients, and once they are off the streets, maintain regular contact and provide ongoing support to ensure a sustainable pathway.
  • Provide opportunities for the street population, rough sleepers, and homeless individuals to move into sustainable housing, promoting integration into mainstream society.
  • Conduct assessments of vulnerable clients and provide sustainable housing support plans.
