Are you an experienced nurse with a passion for community care? We have an exciting opportunity for an existing nurse within LPT to join our Virtual Ward Programme on a 9 month secondment with a focus on delivering care to patients on the Frailty Virtual Ward.

The Frailty Virtual ward is a joint service between Leicestershire Partnership Trust, University Hospitals of Leicester and Adult Social Care which will provide a safe alternative to hospital for patients living with moderate to severe frailty experiencing an episode of ill health, through community-based acute health and care delivery. Central to this approach is services working toward providing a model that is patient centred and in which home or usual place of residence is an option for care.

Individuals on the Virtual Frailty Ward will be high acuity patients who would otherwise be in hospital because they require in person monitoring and treatment. Patients will be remote monitored (tech and non-tech enabled) which will allow early deterioration recognition and in person care and treatment delivered by a multidisciplinary team.

Main duties of the job

The post holder will be an experienced registered nurse, who will work within the Virtual Ward team. We would encourage applications from professionals with high level clinical skills, knowledge and experience in a constantly evolving environment.

As a Band 6 complex care nurse for Frailty Virtual ward one of your key roles will be supporting the remote monitoring of patients on the caseload. Throughout the secondment you will also be central in leading service development to ensure that the remote monitoring of patients is embedded within existing nursing teams, enhancing the delivery of complex care nursing across the service.

This is a fantastic opportunity to develop, you will be key in the clinical coordination of the team on shift and support the clinical management for patients on remote monitoring. You will have opportunities to develop your clinical skills in conducting remote clinical assessments and treatment for patients across the Virtual Ward pathways. Develop new skills outside of your normal scope of practice to enable a single holistic assessment approach, following established escalation pathways.

The role will primarily be office based within the VW team however the role will also require face to face clinical assessment and treatment to patients. At present the role is core working hours 0800-1800 with weekend/bank holiday working on a rota basis.
