Job summary

Would you enjoy the exciting challenge of becoming a Senior Nurse forComplex Care? If so, we are recruiting now to a vacancy in City West integrated Community Nursing and Therapy hub, based at Braunstone Health and Social Care Centre.

You will be required to work alongside the Senior District Nurse inpartnership, providing leadership and clinical expertise in the management ofpatients on the caseload. You will be involved in the triage of new andexisting patients to the caseload, undertake holistic assessment using advancedclinical skills and coordinate the care of those with complex health careneeds. In addition, you will have line management responsibility for the Band 6Nurses for Complex Care.

Complex patients on the caseload may have multiple co-morbiditiescombined with challenging social or environmental factors which complicate theability to deliver nursing care effectively, whilst maintaining high standards.Therefore, a creative problem-solving approach and determination to deliverquality, patient centred care is essential.

Main duties of the job

In the SNCC role you will be required to:

undertake interventions with clinically complex patients work with the wider multidisciplinary team to plan patient centred, individualised care to support patients with multiple comorbidities to remain at home and avoid hospital admission support patients and their relatives/carers with self-care techniques to maximise independence effectively and efficiently triage patients referred into the service have oversight of the whole caseload, ensuring that caseloads are 'clean' and visiting schedules are appropriate line manage the Band 6 Nurses for Complex Care within the teams in which you are based facilitate meaningful clinical supervision provide assurance and narrative for any non-compliance with supervision and put plans in place to address have oversight of the clinical competencies of all clinical staff and support individuals in development / upskilling where required manage clinical incidents in a timely manner, recognising trends and taking action to implement change and learning be proactive in the implementation of service line Quality Improvement initiatives
