The service you will work in….

Ealing Older People Floating Support (EOPFS) provides support to service users in the community. We work with people with a wide range of long-term conditions and complex needs. We promote our service users' independence whilst aiming to improve their wellbeing and supporting progress towards individual goals.

A day in this role looks like….

As a floating support worker at Hestia, you will play a crucial role in our Ealing Older People Floating Support. Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence.

A day will never look the same and some of the things you can expect to do/be responsible for are:

  • Empower a caseload of clients with a range needs and their carers to manage their health and wellbeing, maintain their independence, identify their strengths and achieve their goals and aspirations, working in partnership with our subcontractors as required in order to achieve this, under the direction of the Team Leader​​
  • To clearly articulate the aims and objectives, pathway and service model to people using the service so they can fully participate in and co-produce their support​​
  • Empower service users to engage effectively in their liaisons with professionals within the community such as social work teams, housing providers, health professionals, JCP etc​​
  • Work flexibly as part of a team to ensure service coverage and that contractual obligations are met​​
  • Develop co-produced group activities, peer support networks and other community-based approaches to empowering service users to achieve their goals and aspirations and to encourage service users to access these opportunities​​
  • Actively monitor the health, wellbeing, safety and security of the service and its users ensuring all concerns are followed up promptly and in line with procedures​​
  • Participate in the core work of the service, providing support to colleagues, carrying out tasks allocated by the service manager and using initiative to identify and deal with additional tasks as they arise​​
  • Ensure that administration is done in good time; that detailed, concise and accurate records are created contemporaneously, and that confidentiality is appropriately maintained​​
  • Carry out duties, in line with organisational policies and procedures​

  • Ability to deliver high quality floating, outreach or accommodation based support services to people with a range of needs and risks​​
  • Good knowledge of welfare benefits and housing options and legislation related to statutory duty​​
  • Knowledge of health and safety relevant to role as a Floating Support Worker and working in Service Users' homes​​
  • An ability to undertake client-focused key working, support planning and risk assessments to maximise Service Users' independence around a range of areas​​
  • Good understanding of the range of issues, diagnoses and treatment relevant to the client group including mental health and substance misuse​​
  • Willingness to actively engage in undertaking relevant personal development and training.​​
  • Ability to travel across Ealing Hammersmith and Fulham
  • Previous experience of using computer-based systems to accurately record work and client details​​
  • Knowledge and understanding of safeguarding issues and ability to address them appropriately
