Coventry

This is an exciting opportunity to build on the existing mental health practitioner offer across the primary care networks and to support GPs in the ‘ Central ’ PCN. This compromises of 4 GP surgeries with a population of approx. 33,000 people.

This is an ideal post for those professionals who are proactive, dynamic, and passionate about innovation and providing a high-quality holistic approach to the care of people who have been identified as in need of mental health support in a primary care setting.

This is an exciting opportunity that recognises that placing specialist mental health staff in GP practices and promoting early assessment and treatment in a recovery focused way will help people with mental health problems achieve the best possible outcomes.

It will include reviewing patients presenting with mental health issues, consider a range of options regarding treatment interventions providing them with highly specialised advice concerning care when appropriate and liaising with GPs and MH teams.

This will include facilitating short term case management offering professional advice, joint support and information. To work closely with social prescribers and other professionals within the PCNs multi-disciplinary teams, to support the patient to access the right services in the right place and at the right time.
