Cardiac Services Mdt Co-ordinator

The role of the Cardiac Services MDT Co-ordinator is to provide a comprehensive administration service to ensure effective management of accurate and timely data to enable clinical decision-making at Multi-Disciplinary Team (MDT) Meetings.

This will primarily involve co-ordinating, collecting, recording, analyzing, interpreting and reporting statistical data which is required by WHSCC and track patients through their cardiac pathways in line with Referral to Treatment (RTT) standards. Also to be responsible for providing an accurate and timely account of the entire patient pathway tracking the progress of patients with cardiac disease from referral, work-up, intervention through to eventual discharge.

To communicate and liaise with key personnel along the pathway. To record and maintain up to date information on all, inpatient and outreach services. To signpost to appropriate services and act as a single point of contact for the specialist nursing teams
