Aos Co-ordinator

The Oncology Multi-Disciplinary Team (MDT) Co-ordinator is a front facing operational role for supporting patients and also clinical staff. The post is based with the Assessment Unit and Ambulatory Care teams as part of the Acute Oncology Service (AOS).

The post holder will work collaboratively as a member of the Cancer Centre Acute Oncology Service and across the South East Wales Health Board AOS teams to ensure a smooth pathway for patients presenting with oncology emergencies, including those diagnosed with a cancer of unknown origin (CUP) and also those reporting toxicities from their Immunotherapy treatments, ensuring that the VCC teams are provided with high level daily support. Both the Immunotherapy Toxicity and CUP services are being developed and expanded and the post holder will play a key role in supporting these services.
- Act as first point of contact for the AOS, CUP and Immunotherapy teams and accurately record and relay messages and act upon them as appropriate.
- Responsible for communicating potentially complex information to colleagues within VCC and external to VCC. This is not limited to but could include; information regarding treatment plans for the emergency care of patients, results of scans, treatment summaries, post treatment care plans.
- Manage the co-ordination of acute oncology admissions through to the appropriate clinical review. The role within the AOS, CUP and Immunotherapy service requires the co-ordinator to collect important and vital data relating to the patients journey and treatment management. This information is then recorded into a cancer database, which is used to monitor the effectiveness of the AOS, CUP and Immunotherapy service.
- Co-ordinate the appropriate recording and processing of complex and confidential patient information, in relation to the Cancer Standards and requirements to demonstrate an effective AOS, CUP and Immunotherapy service. Ensuring that all data sets are inputted into the relevant database, in compliance with agreed national data definitions.
- Assist in the analysis of activity and performance to understand the reasons for under and over performance. Analysis is complex and made up of several components including changes in clinical practice and service change.
