**About the Role**
The role of a Hospital Discharge Coordinator is pivotal in supporting operational social care teams to ensure individuals are discharged from hospital safely and promptly. By fostering strong partnerships between Health and Social Care, the coordinator plays a vital role in facilitating effective discharge processes across all pathways. This role demands a proactive, solution-oriented approach and collaboration with multidisciplinary teams to promote the wellbeing of individuals.
**Key Responsibilities**
- Manage and coordinate a daily position statement, ensuring timely communication to all partners.
- Circulate updates daily or during surge periods as required.
- Utilise active listening, observation, and clear communication to build and maintain effective relationships with teams, partners, and care providers.
- Act as an ambassador for South Tyneside Social Services, promoting collaborative relationships aligned with South Tyneside’s PROUD values.
- Confidently and professionally challenge decisions, address delays, and resolve queries affecting hospital discharge flow.
- Participate in solution-focused discussions to address delays or risks, escalating significant concerns to the Hospital Discharge Manager when necessary.
- Ensure appropriate care and support are in place on the day of discharge. Work collaboratively with partners to minimise delays and enhance individuals’ wellbeing.
- Work closely with colleagues across health, housing, and the voluntary sector to optimise resource utilisation and system-wide efficiency.
- Adhere to safeguarding policies, ensuring individuals are supported in making choices and maintaining control over their lives.
**Applicant Requirements**
- **Qualifications and Experience**:
- A background in health, social care, or a related field, with relevant qualifications (e.g., NVQ Level 3 in Health & Social Care or equivalent).
- Previous experience in a hospital discharge coordination role or a similar capacity.
- Proven experience working within multidisciplinary teams.
- **Skills and Knowledge**:
- Strong interpersonal and communication skills, with the ability to build relationships and work collaboratively.
- Confidence in challenging decisions and addressing delays while maintaining professionalism.
