Organise and support safeguarding meetings/MDTs, including preparing agendas, circulating minutes, and updating records. o Support training and maintain training registers for practice staff in line with intercollegiate requirements. o Support audits and assurance processes for safeguarding activity. o Embed learning from local and national safeguarding reviews.
- Review ED attendances and flag concerns to safeguarding leads. o Escalate concerns when information indicates risk. o Maintain up-to-date directories of safeguarding services and contacts. Long-Term Conditions o Maintain oversight of recall systems for patients with long-term conditions.
- Conduct EMIS searches to identify patients for recall, send invites, and code accordingly. o Standardise recall processes across the network to ensure efficiency and reduce unnecessary contacts. o Ensure monitoring requirements for long-term conditions and high-risk drugs are met. o Smooth workload across the year by managing recall schedules.
- Support achievement of QOF, IIF, Vaccinations & Immunisations, and other Enhanced Service Contract requirements. o Monitor Population Manager achievements and liaise with practices to address underperformance. o Enter and amend SNOMED codes in line with defined protocols. o Prepare patient letters and information leaflets as required.
General o Provide efficient, well-organised administrative and operational support to PCN practices. o Work flexibly across practices, remotely, or in community settings as required. o Undertake other duties reasonably required by the Clinical Director or PCN Lead Manager. It should be noted that whilst this job description lists the main areas of responsibility, there may be additional tasks appropriately assigned by either the Clinical Director or PCN Lead Manager to this role.
